Time Type: Part time Department: PACE Primary Care - Nursing All Locations: 26 Sturgis Street - PACE Winthrop Position Summary: Job Description Summary A Registered Nurse holds ultimate responsibility for direct and indirect nursing care, including providing nursing care, health maintenance, teaching, counseling, planning and restoration for optimal functioning and comfort. Skills include demonstrating a comprehensive understanding of the department’s immediate needs and long term growth goals, showing initiative, using critical thinking to problem solve independently, exhibiting leadership qualities, and effectively engaging with the clinical team. Essential Duties and Responsibilities: Qualifications and Requirements: NeighborHealth opens our doors to people from all demographics and cultures. Pay range: $40/hr up to $60/hr based on experience Responsible for the general oversight, assessment, planning, and construction of the nursing care delivered to the participant. Assess a patient through obtaining history, performing appropriate tests, making or collecting observations from the participant and caregivers, and consulting with other members of the IDT. Teaches patients and families health care regimens. Explains and provides information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients. Safely and effectively administers medications, tests and treatments as ordered. Manages the refills of all patient medications with appropriate authorization by the provider. Provide and coordinate health teaching required by individuals, families and groups to maintain the optimal possible level of health. Completes biannual and status change nursing assessments for an assigned panel of participants in accordance with Care Planning Policy and documents problems, goals & interventions in the participant’s care plan. Utilizes Assessment tools including but not limited to PACE Nurse Assessment and Medication Management to collect data. Baccalaureate degree in nursing preferred. Current Massachusetts license in nursing. Current BLS certification required. Minimum of 2 years experience in ambulatory, home health, hospital or sub-acute setting preferred, equivalent training or related experience considered. A minimum of one year experience working with a frail or elderly population. Prior experience in interdisciplinary team setting a plus. Maintains a professional relationship with all residents. Maintains confidentiality of verbal and written information pertaining to residents, ALP operations and other personnel. Provides ostomy care. Takes weights and vital signs including temperature, pulse, respiration and blood pressure monthly and as requested by nurse, records, and reports results. Provides ambulation assistance as needed. Assists with transfer to and from bed. Reports any change in resident status to nurse immediately. Responds to emergency calls and assists nurse as needed. Reports any accidents, injuries, or complaints to nurse or administration immediately. Delivers supplies and equipment to residents. Provides light housekeeping. Cleans refrigerator, stove or oven, cabinets, and silverware drawers as needed. Vacuums and dusts as needed. Assists with laundry by washing, drying, and folding laundry and putting it away. Strips and remakes bed as needed at least once a week. Cleans the bathroom including bathroom floor, vanity, toilet, and shower or bathtub. Removes trash from resident apartments daily. Assists resident with meal service as needed including meal preparation and cutting up foods, set-up, clearing and washing dishes. Reports any unsafe equipment to nurse or administrator. Maintains the cleanliness and neatness of the wellness office, laundry room, equipment, and inventory supplies. Removes trash from wellness offices and laundry rooms. Abides by established guidelines of the facility and governing regulations. Performs other duties as assigned.
