Emergency Room and Hospital coverage required with weekday and weekend rotation; Required ACLS, ATLS, and PALS certifications or willingness to obtain upon hire. Successful candidates must be licensed or be able to successfully obtain licensure in the State of South Dakota. Certification from an accredited organization is needed or must be obtained prior to employment. Key Responsibilities: Provides primary health care to patients, according to established evidence-based guidelines of clinical practice. Ability to lead care teams to provide high quality healthcare in accordance with Horizon Quality Improvement Plan and meet established goals and benchmarks. Management of long-term care for acute and chronically ill patients, including care coordination and development of care plans and self-management plans.
