Overview Our Cardiac Surgical Intensive Care Unit (CSICU) is a four 4-bed unit that directs its resources principally towards the care of adult and geriatric patients with cardiac, thoracic, vascular and other surgical conditions requiring critical care located on 4 North North. The care of these patients is primarily focused on post-operative recovery and providing complex invasive hemodynamic monitoring. Nurses must exhibit high levels of professional competency. CSU is an 8 bed stepdown unit. The stepdown unit focuses on the care and recovery of patients after cardiac, thoracic, or vascular surgery. Both units are supported by a cardiovascular educator. Overview The Communication Center RN is responsible for coordinating patient admissions and transfers from physician offices and other outside facilities to DCH RMC and NMC. This role is responsible for assuring that patients have appropriate orders and patient status upon admission. The Communication Center RN participates in planning and assignment of hospital beds in conjunction with the Patient Accommodation Staff and Administrative Supervisor. Responsibilities Applies excellent customer service techniques while serving as the primary contact for Physicians who seeks to admit patients to DCH. Uses excellent organizational skills to achieve maximum efficiency in a fast-paced environment of the Communication Center Coordinates and facilitates the transfer of patients from physician offices and other health care facilities to DCH RMC and NMC using established processes and policies. Collects clinical information to help providers in determining the appropriate patient status and level of care. Collaborates with DCH RMC and NMC with accepting physician to arrange proper placement for patient. Serves as a patient/physician customer service liaison. Facilitates prompt bed assignments for newly admitted patients. Facilitates accurate placement of patients requiring special considerations due to infection control needs or other pertinent clinical situations. Facilitates bed assignment to patients in the Hospital Computer System. Collaborates with Nursing Supervisor and/or Manager of Patient Accommodations regarding patient placement issues. Accurately completes Communication Center data. Responsible for accurate and timely completion of patient registration and account data into the Hospital Computer System. Promotes excellent customer relations with physicians, clinical leaders and staff, patients, visitors, and the community. Provides direct feedback to appropriate person(s) regarding clinical issues. Nurses in this area have a 1:6 ratio and value the delivery of high-quality, specialized care for patients and their families suffering from a variety of lung diseases or injuries. This unit takes pride in the collaborative relationships with departments such as respiratory therapy, physical therapy, care management and pharmacy. This specialty requires a foundation of knowledge in general medicine as well as expertise in respiratory skills. Overview Our General Surgical Unit is a 32-bed unit specializing in exceptional postoperative care for our general surgical patients. Nurses in this area have a 1:6 ratio and are trained in all aspects of postoperative care; including pain management, mobility, and bedside procedures. This team will closely collaborate with local family practice physicians, hospitalists, as well as our surgeons. Overview Our Medical Intensive Care Unit-1 (MICU-1) is a 16-bed unit. Our environment is upbeat, challenging, and embraces a strong team collaboration with nurses, Intensivists, and multiple ancillary departments – demonstrated daily by our multidisciplinary rounds. We care for critically ill adult patients who are being treated for Acute Respiratory Failure, Acute Respiratory Distress Syndrome (ARDS), Sepsis, Gastrointestinal Bleeds, Diabetic Ketoacidosis (DKA), Hypertensive Crisis, Acute/Chronic Renal Failure, and post-Cardiac Arrest requiring Targete d Temperature Management. Given the multidisciplinary approach our Intensivists provide 24/7, we commonly treat the most critically ill patients with complex illnesses across the West Alabama region. Our unit is unique because of the broad variety of patients admitted to the MICU, which allows for the development of critical thinking and diverse range of intensive care assessment skills. When patients are admitted to the DCH Rehabilitation Pavilion, located at Northport Medical Center, they continue to receive hospital care and close medical supervision while participating in the rehab program. Every patient receives 24-hour medical monitoring and care by our physiatrist, nurse practitioners/ physician assistants and rehab nurses. Providers round daily during the patients stay. Patient recovery is aided by the latest therapy equipment including body weight support systems for safe recovery of walking; treadmills for maximum repetitions; a Dynavision machine for improving reaction time and visual processing and a simulation vehicle to practice getting in and out of the vehicle for the ride home. Overview The Float Pool is the ideal team for individuals who have experience in one or more areas and enjoy working in a variety of settings. The Float Pool provides the expertise of experienced team members who are prepared to serve in many areas of our organization. This team is composed of Registered Nurses that at minimum have eighteen months of nursing experience in a hospital setting. These nurses must also have experience in the area(s) that they are hired to work in. Following orientation, the Float Pool team members are deployed to staff scheduling needs at RMC and NMC campuses. Overview Our Medical Respiratory Unit is a 42-bed unit primarily specializing in the care of acute and chronic respiratory conditions. Nurses have a 1:6 ratio and provide care to predominately respiratory patients requiring treatment for pneumonia, shortness of breath, COPD or other various lung diseases. In addition, this unit will further provide care to a variety of medical patients with GI issues, diabetes, and occasional stroke and surgical patients. Patients in this area benefit from daily rounds with the entire interdisciplinary team. This unit will soon house a step-down unit for more acute respiratory patients at a 1:4 nursing ratio as well. Overview Our Cardiac Intensive Care Unit (CICU) is a 8-bed high acuity unit that provides complex monitoring and treatment of adult cardiac patients. Overview Under the supervision of the Infectious Diseases physician, the Nurse Practitioner (NP) is responsible for providing direct patient care and complex care coordination to patients in both the hospital and outpatient setting. The NP will serve as a member of a comprehensive team of health care professionals to deliver high quality and efficient care to adult patients being evaluated and treated for infectious disease conditions Responsibilities Performs initial and periodic medical history, physical examinations, and daily assessments as directed by the physician(s) and records findings of examinations and assessments. Provides education to patients, families, and other DCH staff members on the on Infectious Disease conditions and issues. Performs hospital consultations under physician supervision and direction. Responds to daily telephone requests from outside consulting physicians, DCH case managers, DCH social work, clinical staff, and other DCH Health System staff. Writes orders in the medical record for medications based on established written protocol and licensure. Orders and interprets diagnostics laboratory and radiology examinations as appropriate for diagnosis and treatment of conditions. Monitors treatment plan(s) as established by the supervising physician. Documents all patient encounters, records observations, interventions, and actions in a medically appropriate manner in the patient’s medical record. Evaluates and regularly reevaluates patient care; coordinates care planning with patient, family, providers and other disciplines to achieve patient-centered outcomes. Assigns and delegates nursing care to team members in accordance with the patient’s needs and the capabilities of the team members. Applies safety principles when performing care and accurately documents observations, care provided and changes in care plan. Participates in transition rounds, care planning/problem identification and team shift reports. Qualifications Current Alabama RN license required. Current American Heart Association (AHA) Basic Life Support (BLS) Provider training required. Overview The On-call RN is responsible for calls from referral sources, calls related to patient care, and any visits related to these calls including administrative workflow. This position works 7 consecutive days and is off 7 consecutive days. The work schedule includes: 1) Evening call, each weekday (Monday-Friday) beginning at 1630 and ending the following morning at 0800, and 2) Weekend call, beginning on Friday at 1630 and ending Monday at 0800. May also be responsible for any Holiday hours in their assigned week. Unless visiting a patient or completing weekend requirements (hospital list/transfer OASIS), the On-call RN has the flexibility of working from home. They determine patient eligibility for care, complete OASIS assessments, and develop evidenced-based individualized care plans following established professional standards of care, Agency policies, State laws, Federal laws and regulations. The On Call RN is integral to optimal quality and process outcomes. Responsibilities Responds to all answering service calls: Communicates with patient/family. Establishes positive working relationships with physicians and physician’s office staff and keeps them informed of patient’s problems and progress towards goals as appropriate Assists patient/family (phone or visit) to prevent exacerbation of condition and/or prevent avoidable emergency room visit or hospitalization. Provides the Clinical Managers with a summary of all activity, interventions, and recommendations. Responsible for performing chart audit activities as assigned. Provides skilled nursing care: Revises the plan of care as appropriate, based on patients identified needs and progress toward goals. Reviews and updates the medication list. Communicates activities related to patient care with the RN Care Manager. Assess patient’s outcomes and offer suggestions on ways to improve outcome scores. Completes all required documentation in the EMR. Checks the DCH hospital list on weekends (Saturday and Sunday). Completes a Transfer OASIS on patients admitted and remaining in the hospital for 24 hours. Communicates all findings with the Administrative RN/Manager on-call. Completes visits of any type assigned. Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness/es. At the initial assessment, explains patient’s rights and expectations, HIPAA as it relates to confidentiality of medical information and OASIS data, advance directives, and informs the patient and family of expected disciplines and frequency. Reviews changes as they occur throughout the episode of care. Develops a plan of care that corresponds with physician orders, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family in the planning process. Makes revisions to the plan of care as patient status and needs change. Communicates as needed with and updates the primary physician when necessary. Re-assesses patients within 24-48 hours following a change in condition or discharge from an inpatient setting and updates the plan of care. Provides supervision of LPNs and home health aides per regulatory requirements, Supports and promotes Agency policies and practices. Maintains communication with team members. Maintains a continuing knowledge of competencies related to the Nursing profession by participation in Agency sponsored education and training as well as personal growth through independent study Attends mandatory Agency meetings and in-services. All other duties as assigned. Qualifications Registered Nurse with a current Alabama license. Two years of registered nurse clinical experience required with at least 6 months of home health or hospice experience. Prefer extensive clinical knowledge and experience in the care of patients with multiple and complex diagnoses, disease processes and care needs. Prefer Working knowledge of OASIS and working knowledge of computer systems. Excellent coordination and communication skills. Detail oriented and able to work with minimal supervision. Must be able to read, write legibly, speak, and comprehend English. Must have current BLS certification or be able to obtain within 60 days of employment and before first independent patient visit. WORKING CONDITIONS Is able to lift 50 pounds, turn and position patients weighing over 200 pounds. Ability to stand, walk, kneel, sit, crouch or stoop with good balance for extended periods of time. Must be able to maneuver over a variety of uneven surfaces including fields, grass, ditches, gravel, concrete, wood, vinyl, or carpet. Good manual finger dexterity. Must have good communication skills. Must be licensed and insured to operate a motor vehicle. Must have tolerance in a variety of environments to include the presence of pets and infestation.
