Collaborate with PRC staff, providers, and external facilities to ensure services align with CMS criteria . Maintain and track statistical data on consults, monitor deferred referrals, and contribute to PRC continuous quality improvement initiatives . Serve as a clinical resource, helping optimize patient outcomes and streamline referral processes. Qualifications: Graduate of an accredited Associate (ADN) or Baccalaureate (BSN) program in Nursing (recognized by the U.S. Department of Education). Active, unrestricted RN license in any U.S. state or territory. Minimum of 3 years of RN experience , with at least 2 years in case management and PRC-related work . Basic Life Support (BLS) certification (American Heart Association). U.S. Citizenship required . Certifications: Basic Life Support (BLS) through AHA required; Certification in Infection Control (CIC) strongly preferred or willingness to obtain within one year of hire.
