Valid PA RN/GN License JOB DESCRIPTION: Utilize nursing knowledge and skills in the safe implementation of preventative therapeutic and rehabilitative nursing care of assigned resident as evidenced by documentation and observation of positive resident care outcomes. Performs independent nursing functions using established procedures, policies, guidelines and standards. Admits and assess residents including initial care plan. Administers assessments, treatments, medications, and diets accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes. Completes all required resident charting every shift. Makes rounds on unit every shift; assess orderliness, infection control and resident care Observes and documents changes in residents’ condition and report to the attending physician and other departments. Participates in discharge planning with physician, and other departments according to residents’ needs. Directs, supervises, and assists CNAs and other nursing staff in providing bedside care. Works collaboratively with other members of the health care team in coordination of the resident’s care as evidenced by other department feedback and coordinates communication among team. Performs other duties as assigned.
