The APRN or Physician Assistant is responsible for assessing physical and psychosocial status of patients in an inpatient and/or outpatient setting. The APPs will see GI and/or Hepatology patients in an outpatient clinic setting by means of interview, health history, physical examination, and diagnostic studies. This position could require inpatient M-F rotation on the UMH Hepatology inpatient service for the right candidate. For outpatient ambulatory work, this would require 7-8 half days in outpatient ambulatory clinics and at Uhealth locations. We have ambulatory practices at UHhealth Towers, UMHC Crohns and colitis Center, Kendall, Lennar, Doral, Sole Mia, Boca, Plantation, Deerfield sites. For Inpatient rotations, APP will round both independently and collaboratively on Hepatology Inpatient Service at University of Miami hospital no later than 8 am and have proper sign-out of patients with fellows at the end of the day and prior to weekends. Recognizes deviations from normal in the physical assessment. Works in collaboration with a physician in formulating treatment plans for health problems and follow-up. Writes prescriptions for medication, blood products based upon laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post-discharge care in accordance with written practice protocols. APP will independently follow patients in an outpatient setting and manage the care of patients concurrently. Initiates and facilitates clinical research protocols and coordinates patient care with research staff. Obtains complete medical history independent of supervision by the physician. See patients in follow-up in between infusions and post procedure as necessary. Initiate and Maintain infusion orders for non-chemo infusions as per protocol, as needed Orders appropriate procedures and laboratory tests according to practice guidelines. Develops, implements and evaluates a plan of care that is appropriate for the patient and family. Actively participates in quality assurance programs and peer review activities. Responsible for providing assistance to physicians in the care and treatment of patients. Provides medical support to staff, triaging and implementing care in a knowledgeable and skillful manner, as evidenced by team feedback and documentation in medical records. Administers nursing assessments to include: performing examinations, giving injections and assessing a patient's health status. Assess severity of patient's condition, determining and directing patients to appropriate medical providers. Provide peer-to-peer reviews with insurance carriers in support of patients medications Consents patients for GI procedures, and places orders for procedures and preps Maintain EMR inbox and close encounters in a timely fashion according to UMMG guidelines. Respond to patient inquiries and requests, and communicate results to patients in coordination with our back office Nurses, Medical Assistants and Secretaries. Provides inbasket message coverage for Providers who depart the practice, or are out on FMLA. Closes inbasket messages within 7 days Closes encounters within 3 days of visit. Selecting, obtaining, ordering, or administering pre-anesthetic medications. Documenting the pre-anesthetic evaluation and obtaining informed consent for anesthesia. Anesthesia Induction, Maintenance and Emergency: Obtain, prepare, or utilize all equipment, monitors, supplies and drugs used for the administration of anesthesia; perform or order safety checks as needed/ Selecting, obtaining, or administering the anesthetics, adjuvant drugs, accessory drugs, and the fluids necessary to manage the anesthetic, to maintain the patient’s physiologic homeostasis, and to correct abnormal responses to the anesthesia or surgery. Performing tracheal intubation and extubation; placing supraglottic airway providing mechanical ventilation. Managing regional anesthetic techniques including, but not limited to subarachnoid, epidural, and caudal blocks; plexus, major and peripheral nerve blocks; intravenous regional anesthesia; transtracheal, topical and local infiltration blocks. Performance of any block requires special demonstration of competency and is conferred individually on an annual basis by the Chief of Service at the site/ Performance of any plexus, major and peripheral nerve block also requires approval on any annual basis by the Chief of Regional Anesthesia in addition. Providing appropriate perianesthetic invasive and non-invasive monitoring utilizing current standards and techniques and responding to abnormal findings with corrective action Recognizing and treating cardiac dysrhythmias through the use of perianesthetic electrocardiogram monitoring. Recognizing abnormal patient response during anesthesia, selecting and implementing corrective action and requesting consultation whenever necessary. Managing the patient’s fluid, blood, electrolyte, and acid-base balance. Evaluation patient response during emergency from anesthesia and instituting pharmacological or supportive treatment to ensure the adequacy of patient recovery from anesthesia and adjuvant drugs. Providing anesthesia care consistent with infection control and anesthetic safety principles in order to prevent the spread of disease and prevent harm to the patient, the anesthetist and others in the anesthetizing environment. Documenting as part of the medical record, all aspects of anesthesia-related care in thorough and timely fashion. Post-Anesthesia Care: Providing a thorough report of the patient’s psychological and physical condition, perianesthetic course and anticipated problems to the post-anesthetic nurse and physician who assume the patient’s care following anesthesia. Providing post-anesthesia follow-up and evaluation of the patient’s response to anesthesia and for potential anesthetic complications, taking appropriate corrective action and requesting consultation whenever indicated. Initiating and administering respiratory support to ensure adequate ventilation and oxygenation in the immediate post anesthesia period. Initiating and administering pharmacological or fluid support of the cardiovascular system during the immediate post anesthesia period as necessary to maintain appropriate physiologic parameters. Perianesthetic And Clinical Support Function: Inserting central intravenous catheters or pulmonary artery catheters requires special demonstration of competency and is conferred individually on an annual basis by the Chief of Service at that site. Inserting Peripheral venous catheters. Inserting arterial catheters and performing arterial puncture to obtain arterial blood samples. Identifying and managing emergency situations, including initiating or participating in cardiopulmonary resuscitation that involves airway maintenance, ventilation, tracheal intubation, pharmacologic cardiovascular support, and management of blood, fluid, and electrolyte and acid-base balance. Providing consultation and implementation of respiratory and ventilator care. Initiating and modifying pain relief therapy utilizing drugs, regional anesthetic techniques or other accepted pain relief modalities as directed by attending anesthesiologist. Selecting or prescribing medication and treatment modalities related to the perianesthetic care of the patient, with consultation or collaboration when appropriate. Accepting additional responsibilities appropriate to the practice setting, which are within the expertise of the individual CRNA. Participates in the development and update of patient care policies and emergency procedures. Ensures hospice services are available 24/7. Follows state regulatory requirements for supervision of nursing services. Manages all members of the IDT including aspects of scheduling, productivity, mentoring, pay practices, time keeping, performance reviews, and colleague support. Models desired behaviors including timeliness, punctuality, attendance, collaboration, open communication, and equitable treatment. Actively collaborates regarding QAPI activities and provides feedback and input into Performance Improvement. Participates in performance improvement programs. Promotes hospice philosophy. Upholds a personal philosophy of integrity and commitment; communicates proactively when schedule changes are needed. Analyzes and reports on quality metrics including Hospice Item Set, CAHPS, and symptom management outcomes. Establishes departmental goals consistent with Moments Hospice goals and objectives. Supervises patient and family care as specified by the plan of care, assessing appropriateness, continuity, service, and quality of care. Models and facilitates communication between colleagues including medical records, business office, intake, clinical team, and leadership. Participates in care delivery for patients as needed including conducting admissions, recertification visits, routine visits, and discharge visits. Manages the care for a caseload of patients if needed to support ebb and flow of census changes. Participates in after-hours shifts as needed including primary and back-up on-call. Serves as the administrator on call when scheduled. Make and/or delegate post-admissions calls. Directs responsibility for the management of Pharmacy, Medical Supplies, Durable Medical Equipment, Mileage and Labor for program to meet budget expectations. May be designated the administrator or alternate administrator for the licensure of the program based on state requirements. Ensures maximum utilization of resources. Promotes a positive and collaborative culture. Performs other duties as assigned. * Education and/or Experience Registered Nurse with minimum of three years in Healthcare required, preferably in Nursing Facility, Home Health or Hospice setting. Bachelor's degree strongly preferred. Experience in leadership or management strongly preferred.
