Summary: The Registered Nurse (RN) plays a critical role in executing InterCommunity mission to promote whole person heath. Under the supervision and direction of the Residential Nurse Manager, the Registered Nurse is responsible for providing alcohol and drug focused nursing assessment at admission to the program, medication administration, and ongoing client health monitoring, including treatment coordination. Essential Duties & Responsibilities: Timely administer medications to patients and monitor patients for reactions or side effects. Maintain accurate, detailed reports and records. Complete focused alcohol and drug assessments at admission to the program. Monitor, record, and report symptoms or changes in patients' conditions. Order, transcribe and reconcile medications per MD order as needed. Facilitate effective crisis intervention. Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans to include referral to higher level of care. Modify patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes. Direct or supervise less-skilled nursing or healthcare personnel. Refer patients to specialized health resources or community agencies furnishing assistance. Perform other duties as required. RN Revisit Nurse Masonicare Home Health & Hospice - Norwich, CT area Pay-Per-Visit The Revisit Nurse ensures quality and safe delivery of nursing and home health services rendered. He/She coordinates provision of home health care services that reflect the agency philosophy and standards of home health and nursing care of assigned adult, geriatric, pediatric, and neonatal clients. Prepare patients for examination, which includes conducting interviews to verify patient information, record medical history; confirm purpose of visit Perform preliminary physical test (blood pressure, weight, temperature, etc.) for vital signs and escalate critical cases to provider as needed. Conduct point of care testing as needed (i.e., flu, strep, urinalysis, ECG, HCG); and provide patient information to provider. Responsible for stocking supplies and maintenance of exam rooms, front desk, and reception area; ensure providers have medical supplies needed. Responsible for completing order sheets to maintain supply inventory levels; regularly check expiration dates on supplies. Responsible for maintenance of medical records and scanning charts to patient files. Answer phone calls, collect and sort daily incoming and outgoing postal correspondence Ensure reception is well maintained Conduct courtesy call-backs for patient follow-up. Perform opening and closing duties, which includes cashing out, end-of-day communication, and securing the building. Follow operating instructions to perform and document daily controls and calibration of equipment; maintain Quality Assurance/Quality Control logs for equipment troubleshoot breakdowns, perform preventive maintenance, and submit repair tickets as needed.
