Contingent Hospice RN Case Manager position summary Our Hospice RN Case Managers provides professional nursing care to the organization’s hospice clients as prescribed by the physician, and/or requested by the client/family, and in compliance with the state’s Nurse Practice Act, any applicable licensure/certification requirements, and the organization’s policies and procedures. The RN is fully responsible for the client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes, is designated as the case manager for hospice patients. The Hospice RN in collaboration with the physician and other Hospice team members anticipates and manages patient’s symptoms, assists in identifying family/caregiver grief/loss issues and assists in implementing the Hospice plan of care. Provides functional support to Hospice Aides and LPN/LVNs as needed. Territory: Athens, GA Responsible for case managing a team of hospice patients. RN case manager will collaborate with interdisciplinary team and attend weekly Interdisciplinary meetings. RN Case manager will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms. Position is Contingent primarily Monday-Friday when needed, with occasional weekend coverage when needed. Employment Type: Part time Shift: Night Shift Description: Position Purpose: The St. Mary’s Neonatal Intensive Care Unit (NICU) provides a individualized, compassionate healing environment by caring for patients ranging from critically ill premature infants to full-term infants with minor illnesses. What you will do: Our highly trained team attends every C-Section and high-risk delivery and commonly treats respiratory distress, sepsis (overwhelming infections), hypoglycemia (low blood sugar) and complications of prematurity. The expert care provided by St. Mary’s NICU allows families to keep their babies close by and in the same building as the Family Birth Center. Under limited supervision, the Registered Nurse is responsible for assessing patients, including developing, evaluating, and modifying the care plan. The RN interprets and performs complex patient-care procedures specific to their expertise. RNs are members of the Care Team and Support Team and have accountability to function as a team to accomplish patient outcomes identified in the pathway/plan of care. The Registered Nurse is a professional caregiver who assumes responsibility and accountability for the care given to a group of patients for a designated time frame. The RN provides care to those patients via therapeutic use of self, the nursing process, the environment/technology, and other healthcare team members, according to established standards. Minimum Qualifications: A current license to practice as a Registered Nurse in the State of Georgia. Graduation from an accredited nursing education program approved by the Board of Nursing or found by the Board to be substantially equivalent to the programs in Georgia.
