The NP works collaboratively with the clinical team and in close conjunction with the HPM Physician to diagnose, prescribe, and conduct ongoing assessments. The NP is a conduit for a patient’s ever-changing medical information, helping ensure each person can get the care they need, when they need it. Responsibilities The Nurse Practitioner (NP) provides quality patient care in variety of settings in collaboration with and under the direction of the Hospice and Palliative Medicine Physician. Follows established nursing and organizational standards, procedures, and practice. Gives specific patient care directions to nursing and other staff. Provides education and support for patients, families and staff members. May be asked to rotate to other regions/departments. Qualifications Experience Requirements: Must have 5 years of recent clinical experience to include sufficient experience in hospice, palliative care, and/or acute inpatient care. Education Requirements: Must have a Master's degree from an accredited program for Nurse Practitioners. Required Certificates and/or Licenses: Must have medical practice license from the governing body for medical practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia. Admissions Nurse

Overview The Admissions Nurse in a home setting, provides and arranges for hospice care for patients in a collaborative relationship with members of the Interdisciplinary team. The Admissions Nurse makes initial nursing assessments and treatment decisions based on the needs and wishes of the patient and their family. In addition, The Admissions Nurse gathers important contact information, reviews and documents the patient’s medication list and takes their full medical history. Location: Washington, DC Hours: 5 8 hour shifts, set or varied schedules available. Some weekend shifts and two holidays per year required. COVID-19 vaccine required to be completed upon start. Responsibilities The Admissions Nurse, in agreement with public health nursing standards, practices, and policies, will provide and arrange for Hospice care for patients/families. The Admissions Nurse is responsible for supervising LPNs, CNAs, Unit Secretaries, and Volunteers. May be asked to rotate to other regions/departments. Qualifications Experience Requirements Must have one year of staff nurse experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly preferred. Education Requirements Must have a degree in Nursing from an accredited nursing program. A BSN degree is preferred. Required Certificates and/or Licenses Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from a compact state is also accepted. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. A CHPN certification is a plus. Nurse Practitioner

Overview Nurse Practitioner – is a member of the active medical staff of Capital Caring Health and provides services to our patients throughout our continuum of care. This person has advanced knowledge and skills to help prevent and relieve suffering some patients experience with life-limiting, life-threatening and terminal illnesses. The care provided includes, but is not limited to, home and facility-based hospice, home and facility-based palliative care, inpatient hospice, hospital-based palliative care consultation, and outpatient palliative care clinics. Location : Washington, DC - Capital City Nursing Home Schedule: Monday - Friday 8:30am - 5:00pm COVID-19 vaccine required to be completed upon start. Expertise in the medical care of terminally ill individuals required. License Licensed to practice medicine in the Commonwealth of Virginia, State of Maryland and/or the District of Columbia as dictated by job responsibilities and regional assignments. DEA and State Controlled Substances Registration in above jurisdictions or Licensure in the jurisdiction of practice - Current Commonwealth of Virginia, State of Maryland, or District of Columbia professional Nurse Practitioner. Skills Training and experience in the palliative care of terminally ill patients. Ability to obtain admitting privileges at area hospitals and nursing homes as requested. Ability to work cooperatively with a team of healthcare professionals throughout the organization. Ability to effectively communicate with patients, families, team members, and professionals in the community served. Ability to remain flexible and adaptable to change despite increased pressure and deadlines. Ability to see, hear, speak, grasp, feel, walk and lift or otherwise perform essential functions with or without reasonable accommodations. Computer Skills Ability to use Microsoft Windows 2000 or XP. Ability to use all Microsoft Products: Word, Excel and PowerPoint. Knowledge of Access Database is helpful. Experience with Suncoast Software is preferred. Physical, mental, emotional Lift and move equipment and patients weighing over 50 pounds. Provide care for highly emotional patients/families. Develop coping mechanisms in order to care for patients who are terminally ill and to work in a stressful environment.
