Job Description General Summary Under limited supervision, this role is responsible for the assessment of patients including the development, evaluation and modification of the plan of care. Provides care to those patients via therapeutic use of self, the nursing process, the environment/technology, and other healthcare team member, according to established standards. Interprets and performs complex patient-care procedures specific to their area of expertise. Role functions as members of the Care Team and Support Team and have accountability to function as a team to accomplish patient outcomes identified in the pathway and plan of care. Patients served include all age groups (Infant through Older Adult as identified in Performance Standards). Principal Responsibilities And Tasks The following statements are intended to describe the general nature and level of work being performed by the person assigned to this classification. Accountable to peers for the advancement of group practice on the unit, and to the nurse manager for the totality of work performance. Collaborates with interdisciplinary team to plan and coordinate care addressing patient/family needs. Serves as clinical resource as needed. Participates in implementation efforts addressing Nursing Sensitive Quality Indicators (NSQI). Complies with standards and supports activities for regulatory compliance. Supports leader and charge nurse decisions. Serves as an effective primary coordinating nurse as required. Models practice consistent with unit operations to ensure safe, timely effective, efficient, equitable, patient-centered care. Follows and supports the AACN guidelines for Healthy Work Environment (communication, collaboration, effective decision making, recognition, leadership, staffing); participates as team member focusing on elements within the Healthy Work Environment. Performs other duties as assigned. Patient Safety Ensures patient safety in the performance of job functions and through participation in hospital, department or unit patient safety initiatives. Takes action to correct observed risks to patient safety. Reports adverse events and near misses to appropriate management authority. Identifies possible risks in processes, procedures, devices and communicates the same to those in charge. Qualifications Education & Experience Licensure as a Registered Nurse in the State of Maryland or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing required. Associate’s degree required. BSN preferred. One (1) year previous RN experience required. American Heart Association (AHA) BLS required upon hire. Must meet all unit-specific certification requirements (PALS, ACLS, NRP). This may include patient care assessment, patient care planning and implementation, and working on patient care quality improvement. This position works in collaboration with the Program Director and the Medical Director to meet the Center’s program objectives and help ensure optimal healing outcomes and patient satisfaction. All clinicians working in a Healogics Center must perform their job responsibilities according to all Healogics policies, Hospital policies, as well as to accrediting organizations, federal and state regulation, and to the Centers for Medicare and Medicaid Services (CMS) guidelines, as applicable. Essential Functions/Responsibilities: Proficient in all responsibilities related to Case Management. May be required to provide case management to a group of patients providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management. Organizes and prioritizes clinical responsibilities, provides clinical oversight to clinical staff, such as Registered Nurses (RNs); Licensed Practical Nurses (LPNs) or Licensed Vocational Nurses (LVNs); Certified Medical Assistants (CMAs); Hyperbaric Oxygen Technicians, etc. Collaborates with other health care providers, key hospital personnel, Wound Care Center Providers, Program Director, and Medical Director regarding clinical and patient needs. Consistently looks at current processes and procedures, and identifies and implements areas of improvement, while complying with Health Insurance Portability and Accountability Act (HPAA), the Department of Health (DOH), as well as hospital and Healogics requirements Engages in staff development, providing general in services as well as individual instruction and orientation.
