Duties Description Do you have nursing experience working in a community setting? Manhattan Psychiatric Center is recruiting a Registered Nurse 2 Community to work with individuals with mental illness to prepare them for discharge to a skilled nursing facility. Candidates from outside or within State Service can apply. As Registered Nurse 2 Community at Manhattan Psychiatric Center, you will be part of the Discharge Planning Team as the nurse expert and duties will include: Coordinating meetings with family members to address barriers related to referrals and admissions of individuals to skilled nursing facility Developing and overseeing agreement between Kingsboro Psychiatric Center and skilled nursing facility to provide convalescent care Conducting site visits to skilled nursing facilities Coordinating treatment and developing individualized care plans Identifying other supports and personal items to assist the individual living at the skilled nursing facility Minimum Qualifications Candidates from outside and within State Service may be considered for hire under the Hiring for Emergency Limited Placement - Statewide (NY HELPS) program as a Registered Nurse 2 Community if they have: License and current registration as a registered professional nurse in New York State, and 2 years of post-licensure clinical nursing experience;1 year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.
