Overview Dialysis is located on the forth floor of DCH Regional campus and the main floor at DCH Northport campus. Our hours at the Regional campus are Monday-Saturday 6:30-1900 with call taken on Sunday and after hours. On the Northport campus hours are Mondays, Wednesdays, and Fridays with call taken on Tuesday, Thursday, Saturday, Sunday and after hours. Our Regional unit has eight dialysis bays, and the ability to perform bedside and peritoneal dialysis. The Northport campus has five dialysis bays and the ability to perform bedside and peritoneal dialysis. The nurses and Certified Dialyses Techs on this unit prepare and perform dialyses treatments on patients within our organization. Responsibilities Provide specialized care to patients who have impaired kidney function and undergo hemodialysis or peritoneal dialysis treatments in the acute care environment. Collaborates and coordinates with physicians and other disciplines regarding individualized patient treatment/care plans by participating in interdisciplinary conferences, care planning/problem identification and shift reports. Overview The Communication Center RN is responsible for coordinating patient admissions and transfers from physician offices and other outside facilities to DCH RMC and NMC. This role is responsible for assuring that patients have appropriate orders and patient status upon admission. The Communication Center RN participates in planning and assignment of hospital beds in conjunction with the Patient Accommodation Staff and Administrative Supervisor. Responsibilities Applies excellent customer service techniques while serving as the primary contact for Physicians who seeks to admit patients to DCH. Uses excellent organizational skills to achieve maximum efficiency in a fast-paced environment of the Communication Center Coordinates and facilitates the transfer of patients from physician offices and other health care facilities to DCH RMC and NMC using established processes and policies. Collects clinical information to help providers in determining the appropriate patient status and level of care. Collaborates with DCH RMC and NMC with accepting physician to arrange proper placement for patient. Serves as a patient/physician customer service liaison. Facilitates prompt bed assignments for newly admitted patients. Facilitates accurate placement of patients requiring special considerations due to infection control needs or other pertinent clinical situations. Facilitates bed assignment to patients in the Hospital Computer System. Collaborates with Nursing Supervisor and/or Manager of Patient Accommodations regarding patient placement concerns. Accurately completes Communication Center data. Responsible for accurate and timely completion of patient registration and account data into the Hospital Computer System. Promotes excellent customer relations with physicians, clinical leaders and staff, patients, visitors, and the community. Provides direct feedback to appropriate person(s) regarding clinical issues. Overview The On-call RN is responsible for calls from referral sources, calls related to patient care, and any visits related to these calls including administrative workflow. This position works 7 consecutive days and is off 7 consecutive days. The work schedule includes: 1) Evening call, each weekday (Monday-Friday) beginning at 1630 and ending the following morning at 0800, and 2) Weekend call, beginning on Friday at 1630 and ending Monday at 0800. May also be responsible for any Holiday hours in their assigned week. Unless visiting a patient or completing weekend requirements (hospital list/transfer OASIS), the On-call RN has the flexibility of working from home. They determine patient eligibility for care, complete OASIS assessments, and develop evidenced-based individualized care plans following established professional standards of care, Agency policies, State laws, Federal laws and regulations. The On Call RN is integral to optimal quality and process outcomes. Responsibilities Responds to all answering service calls: Communicates with patient/family. Is able to push stretcher/wheelchair with average adult patient. Is able to do frequent stooping and on occasion crouching, crawling or kneeling. Ability to reach reasonable distances in any direction. Ability to run in emergency situations. Must be able to move fingers in a coordinated manner. Must have ability to feel and perceive temperature, texture, shape and size with fingertips. Environmental : Includes exposure to human body fluids, disease, infection, lab chemicals and hazard materials and/or cleaning solutions. Requires wearing common protective or safety equipment. Psychological: Includes demonstrating leadership skills, delegating to team members, addressing conflict, communicating with all types of personalities, and being responsible for outcomes/results.
