Position Summary : Responsible for assisting the Corporate Director of Denials Management in the coordination of day-to-day operations and workflow of the Denials team. The objective is to ensure quality outcomes and revenue integrity through coordination, collaboration, and communication with related disciplines to address and integrate opportunities for denial appeals. Essential Functions and Responsibilities as Assigned: Assists in ensuring day-to-day activities of the Denial Appeal department are conducted in an accurate and timely manner. Assists with the coordination of staff assignments and vendor tool management. Assists with writing appeal letters, when indicated, pertaining to RAC/3rd party payers’ disputes/overpayments. Appeal letters will include compelling factual argument and submit to insurance payer requesting timely payment for services. Assists in running reports and utilizes tools (i.e., MHC Data Warehouse, level of care (LOC), Optum, Veracity, CBO MHC Denials Dashboard) for reporting trends and identifies opportunities for potential denials management impact on medical necessity and DRG denials Conducts patient medical record reviews and utilizes clinical knowledge to determine why cases are denied and whether an appeal is required. Confers with physician and management when needed, regarding appeal status. Assesses and presents education opportunities for Denial Appeal staff and physicians. Communicates with and educates all members of the patient care team. Educates health team colleagues about complex clinical appeals/denials, utilization review, including role, responsibilities tools, and methodologies. Required: State licensure as a Registered Nurse (RN) Bachelor’s degree in nursing from accredited educational institution, or actively pursuing degree and to be obtained within five years of accepting position. Three years of recent denial management, case management, clinical documentation, or utilization review experience Five years of clinical nurse experience in an acute care setting American Case Management Certification (ACM) or obtain certification when eligible as defined by the Association Case Management Association, and maintenance of continuing education requirements Preferred: Experience in utilization management/case management/clinical documentation. Performs assessment visit and documents timely. (ie. OASIS/ Skilled Nursing Note etc.). Performs physical examination and review of all body systems and documents accordingly. Develops individualized Plan of Care (POC) to be submitted to the physician for approval and implementation and performs necessary revisions based upon patient needs. Determines medical necessity for other services. Regularly re-evaluates the patient’s nursing needs. Evaluates the patient’s ADL and iADL abilities and need for home health aide. Develops and implements the HHA plan of care when HHA services are ordered. Revises and signs this care plan the beginning of each certification period. Supervises HHA in accordance with state/federal requirements and documents the supervision. Orders “other” professional services that may be appropriate to the needs. Reviews billing processes with patient and/or family advising patient and/or family when co-pay or Medicare is not likely to pay for services. Effectively communicates ongoing with patient and family progression/changes in POC. Effectively communicates with Supervisor/Nursing Director/other disciplines in the case. Communication with the patient’s MD (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities. Actively communicates in the case conferencing sessions to establish best practices. Submits requests for re-authorization of “more visits needed” to payers timely. Coordinates Community Services for the patient to assist in safe home care needs. Participates in the Performance Improvement Committee process. Performs timely clinical record reviews (CRR) per Agency policy in collaboration with the Nursing Supervisor. Participates in ongoing staff meetings/in-services. Participates in the growth of office by being a willing preceptor for same discipline employees. Participates in the planning, operation and evaluation of the nursing services of the organization. Maintains professional licensure per state requirements. Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the event’s occurrence. Submits written time requests 2 weeks or more in advance of planned time off. Other duties as assigned. PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. Department: Nursing Float Pool Daily Work Times: 7:00pm-7:00am Shift: Nights Scheduled Bi-Weekly Hours: 72 Responsible for the delivery of nursing care within the assigned unit scope of practice following hospital policies and procedures, nursing process guidelines, standards of care, and standards of practice. Responsibilities: Assesses each assigned patient, documents and evaluates their response to plan of care, and takes appropriate corrective action. Plans and implements the patient’s plan of care/standard of care and provides patient teaching using the plan of care/standard of care. Oversees all ancillary personnel for technical tasks. Seeks to improve quality of service by continuously developing individual and unit competencies within scope of practice. Integrates into practice the research of best practice benchmarking and clinical pathways, participates and cooperates with ongoing studies and clinical trials promotion and dissemination of research through professional forums including presentations and publications. Required: Graduate from an accredited nursing school. Experience in specialty area will be considered. Experience specific to the unit/position may be required to maintain overall competency level of unit, as required by regulatory agency. Current State of Michigan Registered Nurse license. BLS certification as Healthcare Provider. ACLS certification as Healthcare Provider in the Cardiac Cath Lab. Required after hire into department from an approved provider: ACLS required within 6 months for RNs in Critical Care, Emergency, Endoscopy, 2 East (Orthopedics), Cardiac Cath Lab, Cardiac Rehab, 3 West, 3 Chi, PACU, Same Day Surgery (Penn campus only), Telemetry Floats.; NRP required within 6 months for RNs in Labor and Delivery; PALS required within 6 months for RNs in 5 South (Pediatrics) and PACU; PALS and TNCC within one year for Emergency Department. Oncology Nursing Certification required within 6 months in Radiation Oncology units or within two years in Oncology Services units. Oncology Nurse Society (ONS) or equivalent Chemotherapy/Biotherapy administration course required within 6 months of hire in Oncology Services units. Implements the plan for patient safety, using the patient, family, and community resources. Assists the patient in learning appropriate self-care techniques while delivering quality patient care. Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager. Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician. Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions. Documents medication regimen and updates medication profile according to established procedures. Treats patients and caregivers in the highest and most effective manner. Implement all available actions to prevent avoidable hospitalizations and ER visits. Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel. What is Required? Graduate of an accredited Certificate, Diploma, or Associate School of Nursing Current State License as a Licensed Vocational / Practical Nurse 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
