Participates in team meetings with Medical Director weekly to present all approved new admits, reassessments, hospitalizations, Home Health admits and discharges, and repeat variances. Also, members with potential for discharge, actual discharges, and situations threatening someone’s health or functional status. 8. Attends and participates in mandatory in-services. 9.
