Home Infusion Registered Nurse (RN)

Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in Brazoria and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care. Responsibilities: RN (Registered Nurse) Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System. Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients. Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager. Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation. Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems. Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation. Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. Job Functions: Provides direct patient care as defined in the State Nurse Practice Act, including specialized duties related to end of life care. Implements current nursing practice following a comprehensive assessment and the plan of care. Provide accurate and timely documentation of patient services to reflect the plan of care. Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include Hospice Aide, OT, PT, SW, SLP, Dietician, or Clinical Supervisor. Uses and prepares equipment and supplies effectively, adhering to aseptic technique. Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications. Participates in personal and professional growth and development. Also participates in organization’s quality assessment performance improvement teams and activities. Performs other duties as assigned by the registered nurse. Qualifications: Graduate of an accredited practical nurse or vocational nursing program. Three years’ nursing experience. Community health/hospice or medical/surgical experience is preferred. Currently licensed as an LPN/LVN in the state of practice. Complies with accepted professional standards and practice. Demonstrates good verbal and written communication, and organization skills. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Understands philosophy of hospice concept. Consults with the patient’s attending physician, the hospice medical director and other healthcare practitioners regarding the patient’s eligibility for hospice care. Communicates with other community health practitioners to coordinate the plan of care. Attends and participates in hospice interdisciplinary group meetings as needed. Educates & counsels patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, and spiritual problems. Provides and maintains a safe environment for the patient. Establishes, builds and nurtures relationships with staff and community referral sources to facilitate program growth.
