Job Summary The Pediatric Intensive Care Unit (PICU) is a 30 bed monitored ICU located on the 10th floor of the new C.S. Mott Children's Hospital. The unit is multi-acuity ranging from critically ill to short stay observation. Multidisciplinary care is provided for children from newborn to young adult in partnership with Pediatric Critical Care Medicine, Pediatric Surgery, Pediatric Neurosurgery, Vascular Surgery, Orthopedic Surgery, Otolaryngology, Plastic Surgery, Hematology/Oncology, BMT, Nephrology, Transplant Surgery, and Pediatric Cardiology. State of the art technology including ECMO, Nitric Oxide, High Frequency Ventilation, and CRRT are used in the care of patients in the PICU. Family centered care including family presence during physician rounds, nursing report, and parent spending the night with their child is supported and encouraged. The PICU participates in a nationwide collaborative to improve quality of care and outcomes. Responsibilities* Pediatric Critical Care Nursing is a specialty within nursing that deals specifically with human responses to life-threatening problems. Reviews billing processes with patient and/or family advising patient and/or family when co-pay or Medicare is not likely to pay for services. Effectively communicates ongoing with patient and family progression/changes in POC. Effectively communicates with Supervisor/Nursing Director/other disciplines in the case. Communication with the patient’s MD (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities. Actively communicates in the case conferencing sessions to establish best practices. Submits requests for re-authorization of “more visits needed” to payers timely. Coordinates Community Services for the patient to assist in safe home care needs. Participates in the Performance Improvement Committee process. Performs timely clinical record reviews (CRR) per Agency policy in collaboration with the Nursing Supervisor. Participates in ongoing staff meetings/in-services. Participates in the growth of office by being a willing preceptor for same discipline employees. Participates in the planning, operation and evaluation of the nursing services of the organization. Maintains professional licensure per state requirements. Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the event’s occurrence. Submits written time requests 2 weeks or more in advance of planned time off. Other duties as assigned. About the unit The SICU is a 17-bed unit focused on providing care to critically ill patients who are hemodynamically unstable. We provide the evaluation and treatment of illnesses that may lead to death or acute organ failure, or when there is a concern of imminent physiological deterioration. The SICU patient population includes medical-surgical, surgical, and traumatic injury patients. About the Department Coordinates the care for groups of patients as it relates to disease management, resource utilization, access, discharge planning, quality, service and efficiently across the continuum. This is an interventional role maintaining a patient centered focus. Scope of work Under general direction, integrates cost, quality and utilization to facilitate the admission, continued stay and discharge of the patient. Reviews and evaluates appropriateness of admission or continued stay based on medical necessity. The overall goal of the position is to enhance the quality of patient care and engagement, to promote continuity of care and cost effectiveness through the integration and functions of utilization management, and/or care coordination, discharge planning, and appropriate care transitions. Has accountability for the care coordination and discharge planning of all hospitalized patients. Identifies patients that need care management services (i.e. utilization review; care coordination; and/or discharge/transition planning). Responsible for managing a case load of patients that includes facilitating utilization management, and/or care coordination during the patient’s stay, planning and expediting plans for safe and effective discharge and transition to the appropriate level of care and setting needed after hospitalization. Coordinating care by considering all patient’s needs. Uses critical thinking and effective judgment to determine alternative courses of care. Judiciously uses tools designed to expedite care while being cost effective. Actively participates in readmission initiatives and strategies to maximize patient flow and appropriate resource utilization. Works collaboratively on processes to provide effective transition for patients utilizing hospital outpatient, observation or inpatient services. May review cases for medical necessity, uses InterQual and/or other UR/UM Committee-approved medical necessity screening criteria, when appropriate. Works collaboratively with departmental, revenue cycle, and clinical appeals staff, physicians, and payers to obtain authorization for care and appropriate reimbursement. Determines and assures appropriate status and level of care. Uses defined resources to guide decisions, including Medical Director Care Management, Physician Advisors, and management staff. Routinely communicates with payers, patients/family caregivers, physicians, the interdisciplinary team, post-acute and community-based care providers to facilitate coordination of care and to enhance a seamless transition from hospital setting to the appropriate alternative level of care. Seeks out information and resources to apply creative problem solving for complex discharge/transition planning, quality of care, and utilization management issues. Provides notification and communication to patients/families regarding coverage for hospital and post-acute services, in accordance with CMS regulations. Documents utilization reviews, utilization management actions, care management assessment(s), care plan, discharge plan, and interventions, according to policies, procedures, and regulatory, contractual, and legal requirements. Acts proactively to see that hospital resources are utilized appropriately. Works collaboratively with other departments to define areas of hospital inefficiency and participates in improvement projects.
