Certified Home Heath Aid- CHHA- Hospice

Swan Hospice is looking for CHHA ( Certified Home Heath Aid ) with Hospice experience to join our team. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes. We treat residents, families and loved ones with kindness, respect, compassion and dignity. We have a comprehensive team that can provide support and stability to residents and staff. Job Responsibilities Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry. Feeds and assists with meals. Obtains temperature, pulse and respiration as supervised by the Hospice Nurse. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information , including the date and time of the report and the person receiving the information. Ideal candidates will have: HHA License in the state of NJ Understands the hospice philosophy Minimum of one year experience working in hospice Dependable transportation

Registered Nurse (RN) Hospice Case Manager

Overview R egistered Nurse (RN) Hospice Case Manager works in collaboration with the Interdisciplinary Discipline Team (IDT) to foster a quality experience in accordance with the patient and family’s care plan and wishes. The RN Case Manager plans and manages the nursing component of the hospice caseload as well as the patient's illness and symptoms. He/She re-evaluates effectiveness of interventions and adjusts the care plan as needed. Location: Falls Church, VA Hours: 8:00am - 5:00pm - occasional holiday rotation COVID-19 vaccine required to be completed upon start Responsibilities The Registered Nurse (RN) Hospice Case Manager provides, coordinates, teaches and supervises the nursing care of hospice patients and families/caregivers. The RN Case Manager is responsible for supervising Licensed Practical Nurses, Certified Nursing Assistants, and Patient Care Volunteers. May be asked to rotate to other regions/departments. Qualifications Experience Requirements Must have one year of staff nurse experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly desired. Education Requirements Must have a degree in Nursing from an accredited nursing program. A BSN degree is preferred. Required Certificates and/or Licenses Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from compact state is acceptable. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. A CHPN certification is a plus. Driver's License Must have a driver's license in good standing and access to a vehicle during working hours.

Hospice Registered Nurse (RN), PRN

Overview Hospice Registered Nurse-PRN works as required- based upon the scheduling needs of the team to provide care and support to patients and families. The Hospice RN-PRN ensures that the patients care plan and wishes are being followed. The RN-PRN works in partnership with the IDT team and treats all healthcare needs of the patient including managing symptoms and providing treatment. Overview Registered Nurse (RN) SIGN ON BONUS - Hospice Inpatient Unit – The RN-IPU initiates and maintains the care plan for patients in the acute care setting based on decisions and wishes that have been established by the patient and their families. The RN-IPU focuses upon symptom management while addressing all other healthcare needs. The RN-Hospice IPU works closely with the in-patient team to provide support to patients and their families.
