Assists patients with bath, mouth care, skin, hair and all aspects of personal hygiene. Assists patients with transferring and ambulation as needed. Assists with prescribed exercises which patients and CHHA have been educated in by appropriate health professionals. Prepares meals and assists patients with eating as required. Assists patients in toileting including use of bedpans, urinal and commodes. Prepares household services that will facilitate the patient's self-care at home and are necessary to prevent or postpone institutionalization. Reports changes in patient condition and needs to the Case Manager or Team Manager. Qualifications: High School diploma or equivalent. Completion of CHHA program. 1 to 2 years direct patient care experience in an institution or home care setting. Appropriate understanding of patient care requirements within certification. BLS required. Ability to communicate effectively verbally and in writing. Able to manage time effectively. Bi-Lingual is a plus. Current California driver's license and proof of insurance. We are ramping up our hospice agency and we cannot do so until we have the right CHHA in place! We are looking for candidates who are very flexible with hours, as we anticipate things being slower for the first 6 months (average of 4 hours a week, up to 20 hours a week once we are at a full roster). We are looking for candidates who are comfortable driving within a 50 mile radius of Alhambra. What You'll Do Personal Care: Assisting with bathing, grooming, dressing, toileting, and skin care. Mobility Assistance: Helping patients move in and out of bed, walk, and use assistive devices. Health Monitoring: Checking and recording vital signs (pulse, temperature) and reporting changes to supervisors. Medication Support: Providing reminders and assisting with self-administered medications as directed. We are anticipating an average of 4 hours a week up to 20 hours a week when we are at full roster capacity. The national target compensation for this position is $22-$28/hr. Actual compensation will be determined based on geographical location (current or future), experience, and other job-related factors.

Now Hiring - Certified Home Health Aide (CHHA) This Certified Home Health Aide will be responsible for, but not limited to: Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry. Feeds and assists with meals. Obtains temperature, pulse and respirations as supervised by the Hospice Nurse. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.
