Immediately reports any changes in patients' conditions or incidents to Primary Care Nurse/Director of Hospice Services. Accurately documents services provided in accordance with Hospice policies and procedures on the day services are rendered. Submits documentation in a timely manner. Participates in staff development programs as appropriate for home health aides. Assumes personal responsibility to participate in discipline appropriate education to meet the annual education requirements (12 hours/12 months). Trash and linens are bagged per policy and placed in soiled utility room, cleans and sanitizes all equipment and as needed to include stretchers, wheelchairs, carts, IV poles etc.

Position Summary Assist in the care of Rehab and Transistional care patients with a primary focus on activities of daily living under the direction of a nurse.
