Job Responsibilities

  • Working as an LPN supporting the diverse needs of residents in post-acute facilities
  • Monitoring residents' mental and physical conditions and vital signs
  • Facilitating the intake process, including the documentation of medical histories and any health issues

ESSENTIAL JOB DUTIES/RESPONSIBILITIES: Measures and records patient vital signs; records patient interview and medical history. Catalogs and communicates patient needs to the PCP, Clinician, Specialist and/or other clinical personnel.
