Licensed Vocational Nurse: Home Health

Full-time Licensed Vocational Nurse

Coverage area: Contra Costa County: Walnut Creek*

What you’ll do as a Home Health Licensed Vocational Nurse:

Utilize your outstanding clinical skills as you support and implement the overall plan of care to promote wellness and prevent rehospitalization for patients in their homes.

You will accomplish this with oversight of the assistance and teaching of patients’ and their family/caregivers with wound care, blood draws, tube feedings, Tracheostomy care, foley catheters, Congestive Heart Failure (CHF) management and medication education in the patients home. You will also potentially:

What you'll do as a Home Health Licensed Vocational Nurse:

Work with patients and family one on one to achieve improvement and/or stability.
Case manage patients care episode with interdisciplinary clinical team.
Prepare clinical notes, orders and charting in a compliant time frame.
Collaborate with Case Managers, RNs and agency staff to deliver the best care possible

To be a success Licensed Vocational Home Health Nurse

Current and unrestricted CA LVN license
Current BLS/CPR Certification
Valid CA Driver's License
High level of clinical knowledge in home health care is highly preferred
Experience with high acuity patients is a plus
Experience working with an interdisciplinary team is highly desired
Self-Directed

Outstanding interpersonal skills
Leadership skills
Prior to scheduling an in-person interview, all Candidates are required to complete a computer literacy and competency assessment. This is to ensure you are set up for success and can navigate in our environment including electronic charting, shared understanding of company values and use of our web based communication application. The results of the assessment are confidential and will be used as one of multiple data points included in the hiring decision process.

$75,000 - $160,000 a year

Estimated Wage range Licensed Vocational Nurse: 75,000.00 - 160,000.00 Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match)

* You will be assigned to a Countyas a primary location, however you will be expected to be available to cover for vacancies, leave of absences, as well as supporting increased census and client care needs outside of the assigned area. 21st Century Home Health puts forth every effort to accommodate the assigned primary territory, but on occasion, in order to support safe patient care and meet the demands of a growing business, staff may be reassigned temporarily.
