Job Responsibilities

Delivering post-acute care as an LPN
Monitoring the physical and mental wellbeing of residents and tracking their vital signs

Accesses patient and provider schedules, authorizations, and physician orders to determine appropriate visit schedules under the direction of the Clinical Manager. Assures efficient use of staff by utilizing full-time staff prior to scheduling part-time and per diem staff. Coordinate, assign, and notify appropriate on-call staff and management of the on-call schedule. Work alongside RN Case Managers and others to ensure that patient needs are addressed. This includes ordering supplies, communicating with patients, following up on patient requests, etc. Work with the intake team and branch operations to make contact with new patients. Complete other assigned duties consistent with the nature of this work. Qualifications: Minimum of three years’ previous work experience, with a preference for home health experience. LPN Licensure in state of practice strongly preferred. Demonstrate a commitment to service to others and a willingness to help others in a team-based environment. Knowledge of appropriate communication skills to include the ability to communicate with both internal and external customers and effectively present information one-on-one or in group settings.
