Job Responsibilities

  • Working in post-acute facilities in the area as an LPN
  • Being accountable to follow and adhere to resident care plans
  • Tracking the wellbeing of residents, both physical and mental, and monitoring their vital signs
  • Conducting intakes, documenting health concerns and medical history of residents
  • Administering and properly verifying medications and monitoring their effects
  • Reviewing residents' charts and medical records
  • Assisting with the cultivation of a safe, comfortable facility
  • Maintaining appropriate stocks of medical supplies
  • Participating in IV set up, immunizations, and phlebotomy procedures
  • Helping with the orientation and training of medical staff

Qualifications and Skills

  • Current Illinois state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • High school diploma (or GED/equivalent)
  • Minimum of 6 months of experience (LPN or related role)
  • Ability to lift a minimum of 50 lbs

Nursing is provided through assessment, implementation, and evaluation of the plan of care. Licensed Practical Nurse (LPN) Duties and Responsibilities: Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations. Provides required resident assessments/observations and interventions, reviewing care plans to ensure appropriate care. Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments. Charts nurse’s notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted. Monitors and directs non-licensed ancillary services as deemed necessary to meet professional standards and maintain the highest practicable level of resident care. Communicate/report information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines.
