SNBC Community Health Specialist (Internal Hennepin County applicants only)

Hennepin Health is seeking a Special Needs Basic Care (SNBC) Community Health Specialist to join the SNBC team to provide care navigation for members who are certified disabled focusing on the needs of members with mild-moderate complexity. This position will engage directly with members to offer care navigation and support including establishing primary care, addressing systemic barriers, and facilitating other supportive resources.  

This SNBC Community Health Specialist will work within an interdisciplinary team of nurses, other community health specialists, and a social worker to sensitively address the social determinants of health of SNBC members. This role will focus on a vulnerable population and will work to address their needs in a trauma-informed and member-centric way. Individuals filling this role must be flexible, innovative, and understanding of the challenges and unique health conditions experienced within the SNBC population.

Location and hours:

This position is hybrid and will be performed both on-site at the Health Services Building, 525 Portland Avenue South, 8th floor, Minneapolis, MN 55415, in the field and community-based settings, and remote as job duties require. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 4:30 p.m. While this position is designated as hybrid, based on current requirements, hires must reside in or relocate to Minnesota or Wisconsin.

This is a full-time, benefit earning position. You must be a current regular or probationary employee of Hennepin County to be eligible for this position. Note: Limited duration employees are not eligible.

In this position, you will:

• Provide appropriate follow-up to members while conducting this work in the field, community settings, and over the phone to accommodate member needs. 
• Regularly contact members through various means to ensure needs are being met.
• Manage a caseload of SNBC members with varied disabilities and needs by providing responsive follow up and management of individualized care plans, facilitating voluntary check-ins with members, and engaging with members who may not wish to have a Health Risk Assessment completed.
• Work within MnChoices and Health's Edge Guiding Care, a care management system, to process assessments, care plans, and other relevant care coordination forms. 
• Prepare and maintain records including operational, regulatory, and utilization reports adhering to all contractual requirements specified by the Minnesota Department of Human Services (DHS). 
• Participate in team, staff meetings, and member events and establish and maintain effective relationships with other case management staff to effectively coordinate member care.

Need to have:

• One of the following:
  • Bachelor's degree or higher in public health, nursing, behavioral/social science, or an approved related field.
  • Four years or more of related professional experience providing direct service to assess and support client needs, organizing and reporting on programmatic data, health education, counseling, outreach, and referral services with the community. 

Nice to have:

• Experience:
  • Managing a caseload within a team providing case management or care navigation services in a clinical or community-based setting.
  • Working with Minnesota Health Care Programs.
  • Navigating and connecting residents to Hennepin County social services, including housing programs, financial assistance resources, and community-based supports.
  • Promoting equity and inclusion for people from diverse racial backgrounds, genders, LGBTQIA+ communities, and people with differing abilities.
  • Using Electronic Health Records, MnChoices, Microsoft Office Suite particularly Teams, and Excel, and other related computer applications.
• Knowledge of:
  • Principles and practices of care coordination.
  • Service referral within a county system.
  • Information systems relevant to care management, coordination, and reporting.
  • The unique challenges experienced by those impacted by mental health and chemical health problems as well as the impact of complex trauma history.
  • Trauma informed and harm reduction approaches to care.
• Ability to:
  • Work independently in various settings supporting individuals who are experiencing marginalization and distress.
  • Act in an advocacy role on behalf of members.
  • Ability to function well in unpredictable and challenging situations calling for sensitivity, flexibility, resourcefulness, and independent judgement.
