This role serves on a 24/7 nurse advice and triage resource team for patients of all ages, providing timely guidance and support via phone from an on‑site location. The nurse assesses callers’ symptoms and concerns, offers clinical guidance related to care already received or care that may be needed, and helps patients navigate next steps, including establishing care with new providers when appropriate. This position is not remote.
Schedule:
.6 FTE
7 pm - 7 am
Every third weekend rotation
2 holidays per year
Training will take place over day, mid and night shifts (6-8 weeks)
Minimum Requirements:
Associate's degree in Nursing required
Bachelor's preferred
Required 3 years of nursing (minimum)
Ability to work independently, confidence in one's nursing skills and knowledge
What you will do:
Receives, reviews and acts upon hospital discharge reports showing patients who have not scheduled required or recommended follow up visits
Receives, reviews and acts on Reports for ACO and PCMH
Conducts telephone triage
Conducts an initial assessment to gather and analyze data in accordance with policy.
Identifies patient short -term goals and develops them into an outcome based plan of care.
Evaluates patient response and modifies the outcome based plan.
Educates patient and family to meet identified needs.
Conducts pre-visit chart review of targeted patients
Receives, reviews and acts on Medicaid Health Home Initiative as directed by Clinic leadership
Acts as a collaborating member of the Clinic Medical Home Team and is highly committed to a team construct
Maintains professionalism when assisting callers
Adheres to all safety, infection control and employee health policies and procedures
Based on principles of growth and development, provides care appropriate to the age related needs of the patients served on his/her assigned unit
