Pay Range: $40.57 to $49.59/hour

JOB SUMMARY
LVN responsibility of total resident care for independent and assisted living residents in any area of the RCFE. The LVN performs professional nursing duties in accordance with Masonic Homes and state RCFE regulations. This position reports to the Memory Care Manager and may be required to supervise other staff positions. The LVN will be responsible for residents in memory care and if needed in the RCFE areas when there is only one LVN on shift and/or if the other LVN on shift is unavailable for any reason.
Responsible for staff scheduling, staff mentoring, and provision of ADL care in Traditions Memory Care Neighborhood, functioning as a senior member of the Memory Care Team.
Essential Functions
- Responsible for nursing care, medication administration and treatments for the assisted living residents in compliance with Federal & State regulations and the Masonic Homes guidelines.
- Responsible for the admission of new residents to include all admission assessments, the assessment of resident health status for both new and existing residents, the subsequent development of the service plan and documentation.
- Recognize changes in conditions of residents and notify physicians and/or family members of any change in resident's health and provide proper documentation.
- Performs periodic audits and reviews clinical systems to ensure compliance with State regulations and Masonic Homes policies and procedures as assigned.
- Initiates writing and updating Resident Assessment and Service Plans as resident’s condition changes, in collaboration with active living, social services, and dietary services.
- Supervise resident care personnel on shift in performing duties by checking their work to be sure that assignments have been completed
- Maintains close communication with all departments and staff members, Health Services Director, health care professionals and families about the needs of residents.
- Notifies MD of resident changes and concerns after consultation with RN.
- Personally receive or place calls to physicians, receive and processes MD orders, and order medications as needed.
- Monitors medications of residents as ordered by MD.
- Administer intramuscular, intradermal, and sub-cutaneous injections.
- Chart medications and treatments according to procedure
- Verify inventory of drugs covered by Controlled Substances Act of 1970
- Ensure that individual Resident Assessment and Service Plan is followed
- Attend family conferences.
- Understands role in the Safety & Disaster Plan.
- Documents all care, services, and MD communications accurately and in a timely manner.
- Volunteers for committees and/or task forces as allowable by schedule and approved by manager
- Assists in mentoring and precepting new LVNs
- Participates in facility and departmentwide initiatives as allowable by schedule and approved by manager.
- Takes initiative to offer to do additional tasks, projects, or assignments to further the mission of MHC (Manager approval needed).
- Performs other duties as assigned.
- Organizing and keeping track of resident appointment calendar for on/off unit medical & salon appointments
- Create and update monthly staff schedule for approval by the Memory Care Manager
- Creating and updating monthly resident assignments/ medication pass schedule
- Re-staffing for staff call-offs / PTO’s / emergencies
- Entering schedule into Workday and making updates as needed
- Preparing / updating resident emergency packets including ongoing medication list updates.
- Coordinates moves in/out of the unit (makes work orders, coordinates where furniture will go, makes sure clothing is labeled appropriately & inventory of belongings is completed.)
- Collecting & entering monthly weights/vitals into MatrixCare
- Collaborate with Memory Care Manager on updating resident Service Plans bi-annually, COC’s & inputting changes to the service plan as needed.
- Assists with creating new service plans as needed for new admissions.
- Works with staff to ensure timely food orders and kitchen inventory.
General Responsibilities
- Follows facility policies and procedures and federal / state regulation.
- Participates in facility quality management program.
- Follows oral and written instructions accurately.
- Keeps supervisor informed of work priorities and problems.
- Responds appropriately to emergency situations and disasters.
- Provides a safe environment for patients, visitors and staff.
- Assures security of environment to prevent exit of wandering residents. Implement the facility procedure for elopement if any resident leaves the facility without permission.
- Follows facility standards relating to customer service.
- Maintains confidentiality of residents and other work-related issues.
- Assures that all residents are treated with respect and dignity.
- Follows facility rules of conduct.
- Attends in-service related to work and completes required in-service annually; attends outside classes to enhance role and professional standards, as appropriate.
- Provides timely notification of illness/absences to appropriate supervisor following departmental policy and timeline.
- Attends facility meetings as assigned.
- Follows dress code for work area.
- Performs other duties as assigned.
Skills, Abilities, and Experience
Experience in geriatrics and caring for residents with dementia.
- Excellent customer service and organizational skills
- Sufficient verbal and written communication skills to ensure clear communication with residents, staff and for documentation as required.
- Knowledge of and experience with Excel and scheduling
- Interact effectively with residents, visitors, administration, physicians, volunteers and staff
- Ability to perform nursing care per current community standard for long term care.
- Knowledge of State and Federal laws and regulations governing Long Term Care
- Ability to read, write and speak English in a clear manner
- Ability to understand and respond appropriately to cultural diversity
Educational Requirements/Licenses/Certificate
- Any combination of education and experience that would provide the necessary knowledge and abilities listed.
- Currently licensed as LVN to practice in the State of California
- Basic Life Support (BLS) certification
