Job Summary: The Coordinator of the TAVR (Transcatheter Aortic Valve Replacement) Program utilizes discretion and independent judgment to facilitate seamless care from referral to follow-up for patients who are candidates for TAVR therapy; applies nursing process to the provision of care for this subset of patients; manages and coordinates all aspects of the TAVR program; facilitates a team approach and collaborates with the multidisciplinary TAVR team to establish a plan of care starting from initial interventional evaluation and following the continuum of care including pre-procedure, post-procedure, and continued follow-up post TAVR implant; works with the patient and family to provide teaching, education and support throughout the TAVR process. Serves as a liaison among physicians, staff, patients, families, and payers to ensure effective utilization of resources and coordination of care. Ensures maintenance of established electronic database, and submits patient demographic, medical and progress report information within designated time frames as required by local, state, and federal agencies. Participates in the department quality improvement program and strives to optimize patient outcomes through patient education and clinical expertise. Participates and coordinates TAVR specific LLU-Institutional Review Board approved research initiatives. Performs other duties as needed.
Education and Experience: Bachelor of Science Degree in Nursing required (in progress with quarterly reports acceptable). Master's Degree in nursing preferred. Two years of experience as a cardiac nurse required. ICU nurse experience preferred.
Knowledge and Skills: Able to read; write legibly; speak in English (and Spanish preferred) with professional quality; use computer, printer, and software programs necessary to the position, e.g., Word, Excel, Power Point, Access, CERNER, McKESSON; operate/troubleshoot basic office and patient care equipment required for the position; perform required patient care activities related to the position. Able to relate and communicate positively, effectively, and professionally with others; provide leadership; be assertive and consistent in following and enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English (and Spanish preferred) in person, in writing, and on the telephone; think critically; perform math and statistical functions necessary to the position; manage multiple assignments effectively; compose written material; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail; work independently with minimal supervision. Able to distinguish colors; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place and patient care; see adequately to read computer screens, medical records, and written documents necessary to position.
Licensures and Certifications: Active California Registered Nursing (RN) license required. Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification from the American Heart Association required.
