How you’ll make an impact in this role

Mentor Colleagues: Help junior case managers with complex cases and prioritizing their tasks
Improve Processes: Review case management data to identify trends and enhance workflows
Reduce Length of Stay: Manage patient care to eliminate delays and promote timely discharges
Prevent Readmissions: Develop detailed discharge plans to lower 30-day readmission rates
Act as Advocate: Be the main contact for patients, families, and the medical team to clarify treatment plans
Ensure Continuity of Care: Support patients from admission through discharge to prevent care gaps

What minimum requirements you’ll need

Licensure / Certification / Registration:

Licensed Registered Nurse credentialed from the Oklahoma Board of Nursing obtained prior to hire date or job transfer date required.
Case Manager credentialed from the American Case Management Association (ACMA) preferred.
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

High school diploma/GED with 2 years of experience, OR Associate's degree, OR Technical degree
required. Master's degree preferred.

What additional preferences we're seeking

3+ years of Case Management experience
5+ years of bedside nursing experience
