The Registered Nurse Post‑Acute Care Coordinator assesses and evaluates members' needs to achieve and maintain wellness by guiding members and their families and facilitating access to resources that support appropriate care and overall wellbeing. You will employ a variety of strategies, and techniques to manage members' physical, environmental, and psychosocial health issues. As the Registered Nurse Post‑Acute Care Coordinator, you will identify and address barriers to reduce the risk of hospitalization following emergency department coordination and ongoing patient support, while collaborating with branch case managers and leadership. Work assignments are varied and frequently require interpretation and determination of courses of action. The role requires an understanding of departmental, segment, and organizational strategies and operating goals, including escalation pathways and clinical best‑practice guidelines. You will follow established procedures, exercises autonomy in work methods, and operate with minimal supervision while seeking guidance. You will report to the Quality Assurance Manager.

Required Qualifications
- Active, unrestricted Registered Nurse (RN) license in the state of FL
- Bachelor's degree
- 3 - 5 years of clinical of home health experience
- Experience conducting telephonic assessments, care coordination, and patient education
- Ability to work independently, exercise sound clinical judgment, and make decisions in ambiguous or fast‑paced situations
- Strong collaboration skills with interdisciplinary teams, including case managers and leadership
- Excellent communication, documentation, and organizational skills
- Knowledge of healthcare regulations, privacy standards (HIPAA), and organizational policies
Preferred Qualifications
- 3+ years of clinical nursing experience in post‑acute care, care coordination, case management, emergency department, home health, or managed care
- Knowledge of Wellsky
- Certification in Case Management (CCM)

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$71,100 - $97,800 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
