Position: System Cardiology Clinical Outcome Coordinator-RN
Department: Advance Practice Nurses
Facility: Miami Valley Hospital
Shift: Full-time Days 8am-4:30pm/ 80 Hours Per Pay
The System Cardiology Clinical Outcomes Coordinator is a registered nurse (RN) position for the Premier Health System. The System Cardiology Clinical Outcomes Coordinator is an essential member of the CV Team, which includes physicians and advanced practice providers (APPs), service line leaders, and department leadership. The System Cardiology Clinical Outcomes Coordinator, in conjunction with the System Cardiology APP, will be responsible for managing and coordinating clinical care across the continuum for patients with cardiovascular (CV) disease. The System Cardiology Clinical Outcomes Coordinator helps to ensure patients receive services that are timely, appropriate, and comprehensive and works collaboratively with the members of the healthcare team to facilitate care that is in keeping with the latest standards. Additionally, the Coordinator provides patient/family education and serves as a liaison to support and guide the patient/family through their healthcare experience with the goal to help patients adhere to treatment plans by providing routine communication and helping to facilitate follow up in the outpatient setting. The role requires working with multi-disciplinary teams across the inpatient and outpatient continuum of care and various disciplines. The Coordinator assists in coordinating CV team meetings and case presentation and program development. The Coordinator monitors quality measures and disseminates data to key stakeholders and works with health care teams to implement action plans to help achieve program goals and performance improvement.

Minimum Level of Education Required: Bachelor's degree
