EDUCATION/EXPERIENCE: Bachelor of Science degree in Nursing (BSN) with three years of clinical hospital experience required. Cardiac and Critical Care experience preferred. LICENSURE, CERTIFICATION, and/or REGISTRATION: Current license to practice as a Registered Nurse (RN) in the State of applicable state and Basic Cardiac Life Support (BCLS) certification required. Advanced Cardiac Life Support (ACLS) required. ESSENTIAL FUNCTIONS: 1. Facilitates and ensures that comprehensive, multidisciplinary collaboration of the plan of care occurs with the patient, family, physicians and other health care providers. Coordinates the development, implementation and evaluation of patient focused outcomes through clinical pathways and/or plans of care, analyzes variances and implements strategies to resolve them. Utilizes performance improvement principles to assess and improve quality. 2. Serves as the point person for same day surgery program coordinating care and assisting the patient in scheduling surgery. Coordinates with different services and physicians to ensure all appropriate testing and consents are obtained. 3. Educates patients regarding pre-operative medications, advises Physicians of any possible issues. Promotes positive patient experience through communication and elimination of barriers to care. Develops patient education materials to assist in the education process. 4. Educates patients post-operatively on home care and disease related issues. Promotes continuity of care by establishing and maintaining rapport with regional and internal providers. Makes appropriate referrals to outpatient programs including cardiac rehab programs, PT/OT, Speech Therapy, Coumadin Clinics. 5. Serves as a resource for care providers re quality measures. Assists the department in meeting SDS guidelines by ensuring core/quality measures are met. Assists the providers in the daily plan of care and troubleshooting problems. 6. Screens telephone calls appropriately, in the postoperative period. Provides solutions or resources to community resources available to patients. Assists providers in obtaining needed resources and referrals to outside agencies. Assists providers in preventing readmission by early intervention and troubleshooting problems in the home setting and providing post discharge support to patients and families as needed. 7. Assists providers with appropriates medication reconciliation ensuring that all quality measures are met and patients are discharged on safe medication regimen. Educates patients of medication regimen to assist in compliance and understanding. 8. Assists in maintaining open communication between multiple disciplines by attending daily huddles and sharing of information throughout the day. Attends monthly joint practice committee meetings. 9. Demonstrates personal accountability and compliance with standards of behavior by maintaining a professional appearance. Identifies needs and sets goals for own growth and development, meets all mandatory, organizational and departmental requirements. Performs other duties as assigned or needed to meet the needs of the department/organization. Facilitates the patient's positive progression through the acute episode of care. Carries out the plan of care in collaboration with the patient/family and a multidisciplinary team that includes physicians, staff and appropriate clinical support personnel.
