What is Required?

  • Current LPN license as required by state
  • 1 year of experience in an acute care or equivalent setting
  • CPR certification with American Heart Association or America Red Cross
  • Positive attitude
  • Dedication to quality patient care
  • Strong communication skills
  • Reliable transportation to perform job duties

Implement all available actions to prevent avoidable hospitalizations and ER visits. Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel. What is Required? Graduate of an accredited Certificate, Diploma, or Associate School of Nursing Current State License as a Licensed Vocational / Practical Nurse 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipmen Valid driver’s license and insurance and reliable transportation to perform job tasks You will report to the Clinical Team Manager.
