Responsibilities

As a Home Health Registered Nurse, You will:   

  • Assess/monitor physical, emotional, and psychological needs of patients
  • Create home health care plans that align with MD orders and the patient's goals
  • Direct nursing care: administering medications, treatments, and interventions
  • Provide pain and symptom management
  • Educate and support the patient’s family and caregivers
  • Collaborate with an interdisciplinary team
  • Maintain accurate and timely documentation
  • Participate in on-call rotation as required by the local branch

Qualifications

  • Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the applicable state is required 
  • One year nursing practice in a patient care setting required; and home health, geriatrics or other related settings preferred 
  • Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy 
  • New nursing graduates may be considered in select markets based on program availability 
  • Current CPR certification

The Registered Nurse RN consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role. As a Full Time Registered Nurse, Health Center Group Manager , you'll provide care to client employees and their dependents in our Health Center located in Fort Smith, AR . The scheduled hours are Monday - Friday, 8am-5pm, closed daily for lunch. Strong clinical background, 2 years management experience. Travel required 4-6 times per year to Health Center in Oklahoma City, OK. What You'll Do Oversees the management of single or multiple (2+) health centers and lines of service across contract(s). Manages staff processes typically composed of health center manager, and multiple technical Team Leads as well as indirectly managing all daily operational processes for Medical Directors and clinical providers. Assists in the creation of site-specific goals and measurements related to the needs of the client(s), including management of metrics to Performance Guarantees. Effectively communicates across the various sites and to stakeholders to ensure consistency in messaging. For multiple contract oversight, effectively communicates with DCO to ensure site level priorities are addressed and messaging is consistent across the book of business (BOB). Compiles input and contributes to performance appraisal process for all indirect reports. Responsible for following appropriate processes for hiring, staff development and performance management and provides input to AVP and DCO as needed. Responsible for interviewing, training, and the successful orientation and onboarding of new hires as applicable. Manages daily operations, workflow, work schedules, and timecard monitoring, to ensure efficient and effective health center performance. Identifies process improvement opportunities, workflow efficiencies, determines root cause analysis, creates action plans and presents resolutions and recommendations to the AVP and DCO. Current license as an RN in practicing state. Certification in Occupational Health (COHN/COHN-S) may be required for some sites. Current hands-on certification in AHA or ARC Basic Life Support for health care providers is required. Minimum 3+ years’ experience in the medical field. 2 – 3 years’ management experience. Prior management experience managing a large group a must. Experience accessing CDC, WHO, APIC or other industry standards for Infection Prevention practices.
