Job Description:

LPN:

Provides each Elder with routine daily nursing care and other desired services in accordance with the Elder’s assessment, choices and care plan.  Each LPN is assigned to a primary household of 16 Elders.  The LPN works closely with the Household Coordinator and RN Clinical Manager to manage the day to day functions.

Proficient with performing venipuncture and blood specimen collection. Basic computer skills. Ability to multi-task by managing/working EPIC In-basket, monitoring and directing patient flow, and providing patient care in a coordinated manner at the completion of the orientation period. This includes the ability to manage 14-45 residents upon the completion of the orientation period (Long Term Care only). Access to reliable transportation to travel between facilities if needed based on operational needs. Preferred: 1 year of experience in an in-patient or long term care setting, as appropriate. Familiarity with rules and regulations associated with in-patient or long term care, as appropriate. Physical Requirements: May sit, stand, stoop, bend, and walk intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction.
