Purpose of Position: The advanced practice staff will provide nursing care; education and support to women with breast abnormalities and breast cancer in partnership with other breast care professionals create systems of care which facilitate education, diagnosis, treatment and support for the patient and their families.

Description

A complete history and physical exam.
Initiate consultations.
Manage minor illness.
Stabilize long term illness.
Order laboratory and other diagnostic tests.
Prescribe medications, treatments and devices based on the medication/device protocol jointly developed with a collaborating MD.
NP reports directly to the Director of Radiology and collaborates with the Radiologists.
NP will see patients in the Seifer Imaging Center of Falmouth Hospital.
NP is to keep Radiologist informed of all patients being seen so appropriate physician review and involvement can be insured.
Participates in patient follow up care, including phone calls assigned or requested.
NP will work collaboratively and cooperatively with all physicians, nurses, and ancillary staff members.
Assures patient's rights, as identified by the institution, are met.
Assures that the patient’s environment is correctly maintained and administers patient care in accordance with safety regulations.
Communicates all data relevant to patient and/or family needs to the health care team.
Coordinates patient care with other health team members: Documenting and communicating data for patients with common, well-defined diagnosis/ patient problem to provide continuity of care.
Functions as an advocate for patients and families.
Functions as a liaison between community organizations and the institution for area of clinical practice.
The supervising physician will review all the written notes and physical exams of the advanced practice staff with recommendations for diagnostic imaging exams and procedures.
The advanced practitioner will participate in a quarterly RX review as mandated by MA law.
Consistently provides service excellence to all patients, family members, visitors, volunteers, and coworkers.
Challenges current working practices; identifies process improvement opportunities and presents recommendations and solutions to management. Engages and commits to the organizations culture of continuous improvement by actively participating, supporting, and promoting CCHC pillars of Excellence.
Performs other duties as assigned.

Qualifications

Graduation from a program for nurse practitioner approved by the committee on Allied Health Education and Accreditation (CAHEA) of the American Medical Association
Currently licensed as a Certified Nurse Practitioner in the Commonwealth of Massachusetts
US Department of Justice and Massachusetts’ Controlled Substance DEA Registrations are required for prescribing practitioners.
Breast Health Clinical Navigator (BHCN) certification within 18 months of hire
Current BLS
At least six (6) months practice as a Nurse Practitioner of FNP, ANP, WHENP, preferred.

Nurse Practitioner/Physician Assistant, Cardiology - Office Based

Cardiology Service responsibilities: i) The position will provide Cardiology services as required for patients as assigned, all without regard to source of payment. All such Services shall be performed consistent with all applicable legal requirements and in a professional and cooperative manner. ii) The position will assist in the planning, implementation, and evaluation of Cardiology and related services provided. iii) The position will be responsible for obtaining and coordinating the following general items: history and physical, laboratory results, charting of clinical findings, admissions orders, nursing instructions and other required elements. iv) The position will work closely and cooperatively with the Hospital, the Cardiologists, and other staff across all of the Cape Cod Healthcare system. v) The position will return calls to patients and additional personnel on a timely basis. Place timely calls to patients and additional personnel as needed for ancillary test results, to order follow-up tests, and/or to initiate treatment orders, etc. vi) The position will be immediately available to nurses and other staff for questions and problems relating to patients during the course of their Hospital treatment (whether inpatient or outpatient) as well as on admission and discharge related matters. vii) The position will assist in family liaison.
