Summary:
This position is assigned to work in the Medical Suite with employees, and the entire client population at New Hampshire Hospital.
SCOPE OF WORK: Under the overall supervision of the Infection Prevention Nursing Coordinator, assists in monitoring the infection prevention practice, and provides support, guidance, and consultation to the Nursing staff. Engages in inter and intra-departmental collaboration to ensure continuity of patient care and consistent management practices. Practices within the scope and standards of the Department, Hospital, and Regulatory Agencies.
YOUR EXPERIENCE COUNTS: Each additional year of approved formal education may be substituted for one year of required work experience and/or each additional year of approved work experience may be substituted for one year of required formal education.
MINIMUM QUALIFICATIONS :
Education:Bachelor’s degree from a recognized college or university with major study in nursing, human services, psychology, or a related field.
Experience: One year of experience as a registered nurse in a residential, nursing home, long-term care facility, or similar setting.
OR
Education: Graduation from a recognized nursing program and possession of an Associates degree in nursing, human services, psychology, or a related field.
Experience:Two years' experience as a registered nurse in a residential, nursing home, long-term care facility, or similar setting.

- Current certification in CPR.
- Must be certified by the American Nurses Association or an organization recognized by or affiliated with ANA in psychiatric and mental health nursing, gerontological nursing, wound care, or a specialty related to long term nursing care.

Responsibilities:
- Manages the maintenance of a therapeutic and caring environment, by demonstrating and monitoring staff’s adherence to the policies and standards of New Hampshire Hospital and the New Hampshire Hospital Department of Nursing.
- Monitors and collects patient related data for infection prevention practice and records/reports observations; contributes to nursing assessment, diagnosis, plan and evaluation of patient care through the process of collecting and reporting/recording patient data through interview and/or observations and monitoring. Reports findings to Supervisor and collaborates with Internal Medicine Department.
- Provides staff training and expert consultation regarding infection prevention practices. Monitors and collects data related to infection prevention from contracted employee healthcare provider including, but not limited to immunization status, TB Screening, annual flu shots and Hepatitis B series.
- Schedules, tracks, records, and disseminates information regarding patients outside medical appointments and results. Implements medical treatments as indicated by Nurse Practice Act and hospital standard and as ordered by the physician. Describes, monitors according to protocol, and reports/records evidence of patient problems and response to nursing interventions to assist in formulation and evaluation of the effectiveness of treatment plan.
- Performs employee / patient teaching related to medications, treatments, and health care needs to increase potential for positive outcomes. Is responsible for and actively participates in own skill development via academic course work, seminars, and training programs to improve overall contribution to patient care and professional development.

- Initiates, participates in and supports unit -based and hospital wide nursing research activities. Reviews the high cost of medication against insurance formularies and identify need for medication pre-authorization. Assists with patient care by completing Medical Eligibility Assessments for NH Medicaid Long Term Care Support.
- Acts as model liaison with community, family, and staff in accordance with the customer service values of the hospital. Promotes the use of evidence-based, expert nursing practices and acts as a professional role model for less experienced staff.
Ensures compliance with established hospital and departmental safety policies and procedures by:
- maintaining competency in hospital approved Crisis Management Program (CMP) and Cardiopulmonary Resuscitation (CPR) techniques for adults, children, infants and AED training.
- conducting periodic (minimum quarterly basis) on site reviews
- completing competency-based review, when applicable
- providing on the spot reinforcement/training on safety issues; referring staff to formal training programs, as appropriate; and
- recording safety related initiatives for annual reporting.
- Participates in the formulation of the mission, philosophy, goals, and objectives for the Department; supports same by acting as an agent of the Department through a positive leadership presence in unit based, departmental, hospital wide, or in public meetings/committees within the hospital to support.
- Participates in the process of continuous quality improvement by supporting quality improvement activities; demonstrates commitment to customer service values in professional conduct and by promoting such values with co-workers.
