General Position Summary:
The Licensed Practical Nurse (LPN) utilizes nursing knowledge and skills up to the level of his/her training acquired in a State approved program. The LPN working under the supervision of an experienced Registered Nurse (RN) assists with workplace injury care, regulatory testing, health surveillance programs and employer specific medical services while ensuring compliance with federal, state, and occupational safety regulations.
Essential Functions / Major Responsibilities:
· Performs specific activities within the scope of an LPN as defined by the Arizona State Board of Nursing for employees and clients in the capacity of Employee & Occupational Health.
· Administers medications, injections, immunizations and treatments as per protocol or by direction of the provider.
· Performs health examinations including pre-employment, annual surveillance and return to work evaluations.
· Performs point of care testing, phlebotomy and other clinical skills as needed per protocols or direction from provider.
· Demonstrates computer competency.
· Ensures compliance with HIPAA policies and procedures.
· Communicates concerns regarding patient care and any hospital/departmental operations utilizing the incident reporting system.
· Participates in educational offerings: in-services, and competency activities
· Participates in department/hospitalwide informational meetings: staff meetings, hospitalwide forums, and seminars.
· Promotes the image of nursing in the organization and community in a positive and professional manner.
· Displays proper etiquette and mannerisms that reflect the SHINE Behavior Standards.
· Promotes the Patient Safety Standards as a core value of the organization.
· Knowledge of occupational health regulations including OSHA and DOT guidelines.
· Ability to follow strict chain of custody procedures for regulated testing.
· Works in collaboration with other coworkers with departmental duties, assignments, including clerical and follow up OSHA regulatory standards
Secondary Functions:
· Reviews department and hospital wide policies and procedures annually.

· Requires general nursing skills and knowledge of general office equipment (including the telephone system, fax machine, copy machine, computer, and commonly used hospital programs) as well as excellent computer, communication, critical thinking, problem solving, interpersonal skills, and basic math skills.
· Knowledge of hospital equipment and programs, including all Hospital Information Systems and department specific equipment.
· Recurring work situations with occasional variations from the norm.
· A moderate to high level of complexity.
· Operation from established and well-known procedures.
· Performance of duties with minimal supervision.
· Must be able to read, write, speak, and understand English.
Education and/or Experience:
· Current LPN license in the state of Arizona in good standing.
· BLS Certification (within 30 Days of hire)
Required Certifications (or ability to obtain within specified timeframe):
· Breath Alcohol Testing (BAT) Certification (required within 30 days of hire)
· DOT Drug Screening certification (required within 30 days of hire)
· Certified Occupational Health Conservationist (required within 6 months of hire)
· NIOSH-Approved Spirometry Training Certificate (required within 6 months of hire)
Physical Demands & Job Conditions:
Exert up to 50 lbs. of force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects. The worker is subject to contact with chemicals, contact with infectious fluids, heavy lifting, constant standing, and encounters with upset/disturbed individuals. Ability to wear appropriate personal protective equipment (PPE) when required.
Physical motions include finger dexterity, standing, walking, stooping, crawling, talking, reaching, feeling, sitting, bending, kneeling, climbing, grasping, listening/hearing, handling, lifting up to 50 pounds, and repetitive motions of the hands, wrists, and feet.
