Responsibilities

  • Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care.
  • Completes OASIS assessments timely and accurately.
  • Provides skilled nursing care to patients in accordance with the plan of care and agency policies.
  • Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
  • Initiates appropriate referrals when indicated.
  • Promotes and maintains patient health through teaching and appropriate rehabilitative measures.
  • Assists patients in learning appropriate self-care techniques.
  • Other duties as assigned.

 


Qualifications

  • Current RN license, specific to the state(s) you are assigned to work.

  • One year of clinical experience as an RN. If less than one year clinical experience, you must be approved by VP of clinical.
  • Current CPR certification.
  • Valid driver's license, reliable transportation and liability insurance.
