As a RN Clinical Supervisor You will:    

  • Review new patient referrals and determine care needs; assign appropriate hospice staff. 
  • Support and guide clinical staff through home visits, case reviews, and real-time consultations. 
  • Help develop patient care plans, set treatment goals, and prioritize care. 
  • Lead and participate in team case conferences to ensure coordinated, holistic care. 
  • Conduct regular chart audits and provide feedback for quality improvement. 
  • Participate in hiring, onboarding, and training of clinical team members. 
  • Support staff development and continuing education programs. 
  • Assist in policy development and uphold professional care standards. 
  • Engage in community outreach and promote the hospice’s mission and services. 
  • Contribute to quality improvement initiatives and organizational goals.

Qualifications

 

  • Registered nurse with current licensure to practice professional nursing in the state.  
  • Graduate of NLN accredited school of nursing with a BSN degree is preferred.  
  • Previous experience in hospice/home care setting and with two (2) years management or supervisory experience 
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.  
  • Complies and maintains current CPR certification.

Overview The Registered Nurse (RN) Supervisor provides administrative oversight, nursing leadership and supervision of all patient care areas including off shifts, weekends and holidays. Responsibilities Supervises and gives assignments to the staff assigned to their patients. Oncology Nurse Navigator | Quality | Full Time

Overview The Oncology Nurse Navigator serves as an advocate for patients in accessing treatment services while promoting a positive patient experience for the individual and their family. This position also serves as a role model and resource to oncology nursing staff at UF Health St Johns with regards educational and program needs for complex clinical situations. Responsibilities Serves as a clinical expert in helping cancer patients and their families navigate through the healthcare system. Serves as a patient advocate in expediting patients’ access to hospital services, minimizing delays and ensuring a positive patient experience. Serves as a role model in oncology nursing for Flagler Hospital staff, acting as a resource in oncology nursing, particularly with complex clinical situations by assessing educational needs and planning programs accordingly. Assists with maintaining accreditation requirements for the Cancer Institute. Assists with planning and implementing community outreach activities for the CESC, in collaboration with community organizations including the American Cancer Society. Collaborates with the the Hospital Medical Director, Cancer Center, in identifying and resolving patient care-related issues. Collaborates with the Hospital oncologists, other physicians, staff, and community/advocacy groups to facilitate and enhance coordination of cancer care across the continuum.
