Overview
The Registered Nurse (RN) in the Diabetes Center of Excellence plays a pivotal role in the holistic management and education of patients with diabetes. This individual is adept at understanding the complexities of diabetes management, from medication familiarization to technical troubleshooting of devices, ensuring that patients receive optimal, personalized care in collaboration with the entire diabetes care team.
Responsibilities
Medication Management:
- Possess comprehensive knowledge of all diabetes medications to counsel patients on proper administration techniques, potential side effects, and expected outcomes.
- Prepare prior authorizations for diabetes-related medications for insurance companies.
- Lead the process to appeal denials of medications to ensure patients have timely access to vital treatments.
- Be adept at working with pharmacies to solve coverage issues so patients have safe and effective substitutions of medications where appropriate working along-side the endocrinologist and nurse practitioner.
Education & Counseling:
- Educate patients on the fundamentals of diabetes, ensuring a comprehensive understanding of the disease from both a biological and practical management perspective.
- Foster patient-centered approaches, tailoring teaching methods to individual needs, and ensuring patients grasp the intricacies of managing their condition.
- Conduct glucose tolerance tests, ensuring patient safety and accurate results.
- Offer culturally-sensitive nutritional counseling, emphasizing various dietary approaches, including the Mediterranean diet.
- Provide patient-specific exercise counseling tailored to individual needs and limitations.
- Instruct patients on the implementation, usage, and troubleshooting of continuous glucose monitors (CGMs), integrating these with mobile technology and facilitating data-sharing with the center for remote consultations.
- Upload data from patient’s home monitoring devices so this data is available to the clinical time during visits both in person and virtual.
- Teach foundational elements such as insulin administration, management of high and low blood sugar, sick day rules, and glucagon pen usage.
- Offer in-depth training on different CGM brands to ensure patient proficiency.
Technical & Equipment Support:
- Expertly troubleshoot issues related to insulin pumps, coordinating with distributors for pump authorization, and ensuring patients receive both technical and emotional support.
- Assure on an ongoing basis patients have adequate supplies to avoid interruptions in care.
- Educate patients on pump usage, features, and maintenance.
Collaboration & Team Support:
- Respond to medical advice inquiries regarding diabetes management.
- Collaborate closely with the entire diabetes care team, including the depression care manager, recognizing the importance of identifying diabetes distress and ensuring timely referrals.
- Work alongside the registered dietician and health coach to ensure a comprehensive approach to patient care.
- Uphold and promote excellence in diabetes care in partnership with endocrinologists and nurse practitioners.
Qualifications
Education:
- Graduate of accredited nursing program(s).
- Bachelor of Science in Nursing (BSN) required.
- Master’s in Nursing or related field preferred.
Experience:
- Proven experience with technical aspects of diabetes care, including insulin pumps and CGMs.
Knowledge and Skills:
- Proficiency in electronic health records and medical software.
- Strong interpersonal and communication skills, with an emphasis on patient-centered care.
Licenses, Certifications:
- Current and valid NYS Registered Nurse License.
- Certification in Diabetes Education (CDE) or relevant experience.
Physical Requirements:
- Ability to stand or walk for extended periods.
- Ability to lift, move, or assist patients as necessary.
- Fine motor skills to administer injections, perform procedures, and manage technical equipment.
- Coverage of some evening sessions required.
- Cover two weekends monthly.
