Clinical Manager-Trinity Health at Home

Role Summary:
The Clinical Manager oversees clinical and operational aspects of the homecare program, ensuring high-quality care delivery, staff supervision, compliance, and achievement of key metrics such as clinical outcomes, productivity, employee engagement, and budget targets.

Key Responsibilities:

  • Recruit, retain, and develop staff to meet operational needs.

  • Supervise staff providing direct client care and ensure delivery aligns with standards and physician orders.

  • Manage onboarding, orientation, and continuing education for employees.

  • Conduct performance evaluations and ensure compliance with federal, state, and accreditation standards.

  • Participate in leadership functions and policy development.

  • Oversee client care coordination and monitor records for accuracy.

  • Provide mentorship, feedback, and counseling to staff to uphold clinical and professional standards.

  • Collaborate with interdisciplinary teams to achieve organizational goals.

Qualifications:

  • Strong leadership, organizational, and communication skills.

  • Ability to work with diverse populations, including newborns to geriatrics.

  • Proficiency in computer systems and telecommunication tools.

Minimum Qualifications:

  • Current state license in the applicable discipline.

  • Bachelor’s Degree in nursing or therapy required; Master’s Degree encouraged. (Therapy degrees must meet discipline requirements.)

  • At least three years of experience in a skilled homecare agency.

  • Proven supervisory or leadership experience.

  • Strong interpersonal and communication skills to effectively interact with clients, clinical staff, and team members.

  • Valid Driver’s License with reliable transportation.

  • (California only) Must have one year of professional nursing experience as per regulations.
