Develop, implement and monitor collaborative therapeutic service plans, completing all necessary documentation and reporting significant concerns to the supervisor as needed. Assess individuals’ behaviors and complete daily data, monthly progress notes and quarterly reviews. Foster community integration and provide support to individuals to actualize this goal. Qualifications Bachelor’s Degree; or High School Diploma and two years’ experience working with individuals with developmental disabilities. Strong interpersonal skills and ability to use good judgement. Excellent written and verbal communication skills. Basic computer knowledge. Must hold a valid driver's license, have access to an operational and insured vehicle and be willing to use it to transport clients.
