Job Requirements
The Patient Safety Liaison is responsible for assisting in developing, operationalizing, maintaining, enhancing and evaluating the organizational Patient Safety Program and Culture of Safety, including National Patient Safety Goals strategies, medical error and event investigation and reporting, and coordination of patient safety systems improvements.
DATA COLLECTION AND ANALYSIS: Utilizes effective processes for data collection, evaluation, analysis and reporting of medical error information. Performs and completes thorough, credible and timely analysis of incident trends, sentinel events and serious occurrences as assigned. Conducts intense analysis of high risk incidents and medical errors and facilitates root cause analysis and leads performance improvement teams.
PATIENT SAFETY: Assists in developing, maintaining, enhancing and evaluating the
organizational Patient Safety Program and Culture of Safety. Works with Patient Safety Officer to initiate and develop annual evaluation/ review of the Patient Safety Program. Assists in creating action plans to address national patient safety goals as determined by TJC. Assists in identifying and implementing short and long range goals, objectives and strategies. Develops, reviews and participates in patient safety training. Participates in New Employee Orientation and provides in service programs as needed.
RISK ASSESSMENT: Conducts proactive risk assessment activities utilizing critical analyses and failure mode methodologies. Collaborates with Pharmacy Director regarding management and review of medication error information and potential system improvement. Collaborates with the Directors of Performance Improvement, Risk Management, and the Environmental safety officer and key stakeholders on the development of patient safety policies and procedures.

- Minimum 4 year / Bachelor's Degree: Nursing (Required)5 years patient safety experience
Certification / Licensure / Registration
