The RN Case Manager manages a caseload of patients and families, which includes assessing, planning, implementing, evaluating and documenting the care provided.

Responsibilities

•Identify risks and interventions upon admission, and coordinate with other team members to help address those concerns.•Administer medications and treatments as prescribed by the physician.•Be available, when possible, to meet a patient/family's needs in time of crisis.•Attend and participate in bi-weekly Interdisciplinary Team Meetings (IDT), and provide recommendations on revising the plan of care based on continued evaluation of the patient and family/caregiver needs.

Qualifications

•Graduate of an accredited school of nursing.•Active licensure in good standing as a Registered Nurse through the State Board of Nursing.Minimum of one (1) years’ nursing experience in hospice or home health care preferred.•Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order and a clean driving record.•CHPN certification a plus•Current CPR and Fingerprint clearance or ability to obtain
