The MDS Coordinator we are seeking to fill this position will need to demonstrate leadership on the unit in the presence and absence of the Nursing Director. Live Oak offers care for nursing home residents as well as skilled nursing care. Responsibilities will include, but will not be limited to: directing the initiation and/or implementation of an individually written nursing care plan for each resident; functioning as part of the facility's Administrative Team by delegating tasks, initiating solutions to problems, and by taking part in on-call rotation; hold daily or weekly Utilization Review Meetings to maximize resident's potential and facility's reimbursement potential; provide in service for staff nurses on clinical aspects of Medicare Skilled Charting as needed; works with Rehab department and floor nurses to optimize resident's potential while receiving medical treatment/services; assess resident in order to complete MDS timely in accordance with CMS regulations; works cooperatively with other disciplines to implement POC through discharge. The candidate needs to be kind, compassionate and dependable. An orientation will be provided based on your experience and level of comfort. The individual selected for this position must have a current Louisiana LPN license and will need to have previous MDS experience; Nursing home/LTC experience is a plus.
