PRIMARY FUNCTION:

The Patient Flow Nurse is responsible for continuity of operations within the facility by overseeing all patient admissions, discharges, and transfer activity, while monitoring capacity both currently and prospectively across the TJUH, Inc. Division. Facilitates continuous throughput of patients from all points of entry by utilizing effective verbal communication, swift clinical decision making and problem-solving skills. This includes coordination and communication with key personnel and departments, including Jefferson physicians, nursing personnel, Environmental Services, Patient Transport, JeffSTAT, and the Jefferson Health Transfer Center.

The Patient Flow Nurse operates in a busy, high-stress environment, and must be able to make stressful decisions promptly. The Patient Flow Nurse Manager must be adaptable individual capable of building relationships and managing competing priorities. The Patient Flow Nurse is accountable for defined patient flow quality and safety key performance indicators (KPIs), e.g. ED and PACU boarding, clinical place assignment/placement times, wait times in ED and PACU and adherence to clinical placement guidelines/ unit-based admission criteria

Activities

 Maintains patient throughpout and continually evaluates bed placement progress according to the institution’s Patient Flow Plan.

 Provides clinical guidance to departmental personnel on where to place patients in accordance with unit-based admission and clinical placement criteria.

 Establishes and directs plan for patient placement of all admissions and transfers.

 Collaborates with medical staff and nursing personnel in all departments to optimize patient flow throughout the facility.

 Monitors centralized care progression and quality and safety alerts.

 Maintains awareness of staffing levels in key departments, such as Nursing, EVS, Patient Transport, JeffSTAT, and other departments that might impact patient flow.

 Acts as the Charge Nurse in the absence of an on duty Manager or Supervisor.

Specific Actions/Decisions:

 Targets and assigns beds to patients from all clinical departments, direct admissions, and external transfers based on defined workflow.

 Acts on care progression and quality and safety alerts based on clinical guidelines, protocols and processes.

 Facilitates the Daily Bed Meeting and leads efforts to problem solve during high census periods and develops action plans.

 Sets the priority for EVS and Patient Transport requests to optimize patient flow and reduce wait times.

 Notify Facilities Services of maintenance requests for bed blocks.

OTHER FUNCTIONS AND COMPETENCIES needed in addition to Essential Functions:

 Performs duties in an independent manner based on guidelines set by supervision.

Job Description Nemours is seeking a Sickle Cell Nurse Navigator to join our Nemours Children’s Hospital team in Wilmington, DE The Sickle Cell Nurse Navigator (SC NN) is the key clinical contact for complex patients with Sickle Cell Disease (SCD), their families, and caregivers, in an effort to overcome health care system barriers. The nurse navigator is the primary contact for our patients, responsible for coordination of a high level of service and quality of care to Sickle Cell (SC) patients. This includes coordinating multidisciplinary care, coordination of care between visits, scans, prescription refills, complex scheduling, and sub-specialty interface. This role addresses educational needs, insurance authorizations, and peer to peer for complex patients. The SC NN ensures adherence to patients prescribed Hydroxyurea, patients due/over due for TCDs (transcranial doppler ultrasound), schedules and post patients in preparation for procedures, and identifies patients lost to follow-up. The SC NN partners closely with the Transplant and Cellular Therapy Team (TCT) to provide education regarding Transplant and Gene Therapy as well as performing HLA typing for donor matches. The SC NN is involved with all aspects of the patient's care through the health care continuum and is critical to this at-risk patient population. Essential Functions Improve patient’s access to care by assuring efficient and effective coordination of patient care between visits including scans, complex scheduling, and sub-specialty interface. Job Description Nemours is seeking a full time 0.75 FTE (60 Hours per pay period) Registered Nurse for our 5 West Hematology/Oncology/Transplant and Cellular Therapy Unit in Wilmington, DE. Schedule consists of 60 hours in a 6-week schedule, with the ability to work more hours. 12 hr. Day/Night rotating, holiday requirement, and every third weekend requirement. Flex to other units and hematology oncology clinic as per the unit’s rotation pull guidelines as needed. You must be a graduate of an accredited school of nursing. BSN is preferred Must have Delaware State license (or compact license). Current AHA Healthcare Provider (HCP) Certification required PALS optional but required to obtain within a year of hire. 5 West is a 24-bed unit that has been designated as the Hematology/Transplant and Cellular Therapy and Endocrine unit for the Nemours. This step-down unit provides inpatient care for pediatric patients requiring services from Hematology/ Oncology, Transplant and Cellular Therapy, and Endocrine subspecialties. Admitting diagnoses include but are not limited to leukemia, lymphoma, brain tumors, neuroblastomas, solid organ malignancies, sickle cell disease, bleeding disorders, diabetes, and other types of genetic and immune disorders. In addition, stem cell and bone marrow donors are cared for on this unit. The type of patient care provided in this area is complex due to patient diagnosis, the intensity of therapy, the criticality of patient condition, and the extensive patient/family issues that necessitate nursing intervention. 5 West nurses are responsible for coordinating patient care as part of an inter-professional team. Nurses are required to be competent in the care of the newly diagnosed endocrine patient, and hematology, oncology, pre-, peri-, and post- transplant and cellular therapy patient including the administration of chemotherapy and biotherapy. Competency is established and maintained through an internal educational process. All registered nurses on 5 West must successfully complete and maintain the National APHON (Association of Pediatric Hematology Oncology Nurses) Chemotherapy/Biotherapy Provider Course, the APHON BBMT (Blood & Bone Marrow Transplant) Core Curriculum, Hematology and Oncology 101, and PALS (Pediatric Advanced Life Support) certification within one (1) year of hire. Nurses are strongly encouraged to maintain or achieve their specialty certification: CPON (Certified Pediatric Oncology Nurse), or CPHON (Certified Pediatric Hematology Oncology Nurse).
