Job Summary

The Certified Nursing Assistant or Patient Care Assistant (PCA) as we call it, functions as a member of the patient care team, under the direction of the Registered Nurse (RN), and proficiently and consistently performs all aspects of the PCA role. Responsible for supporting the RN with clinical tasks, including but not limited to blood glucose monitoring, completing EKGs, applying simple dressings, using aseptic technique and responding to medical emergency situations utilizing Heartsaver CPR and AED skills. In addition, the PCA is responsible for responding to the needs of patients and family members and taking an active role in the efficient operation of the unit. Assure the practice of the Patient and Family Centered Care Model in an environment that exemplifies best practice. Prior CNA/PCA experience preferred.

Qualifications

1. PROVISION OF A SAFE ENVIRONMENT

· Anticipates and identifies patient safety risks in collaboration with patient care team and takes appropriate action.

· Utilizes proper body mechanics in moving and assisting patients to prevent personal injury.

· Rounds and returns loose oxygen cylinders to appropriate storage areas.

3. MANAGEMENT OF INFORMATION

· Reviews and is knowledgeable of appropriate policies, procedures, and work rules.

· Utilizes hospital E-mail to access staff meeting minutes, notices, and memos.

· Maintains timely ongoing verbal communication with RN throughout the shift, regarding patient condition including, the status of completing delegated tasks. Recognizes and notifies RN immediately of any significant change in a patient’s condition and/or monitoring alarms

· Accurately collects and records patient data (vital signs, intake and output, blood sugar, weights, restraint care/monitoring, other) and complies with hospital documentation standards to ensure a complete and accurate patient record.

4. COLLABORATIVE INTERDISCIPLINARY CARE

· Maintains and fosters and teamwork approach to patient care.

· Transports patients and equipment throughout the hospital as requested

o Uses appropriate safety techniques and body mechanics when moving patients. Remains with the patient or ensures patient is attended by staff when of other home unit.

5. PATIENT/FAMILY EDUCATION

· Addresses patient comfort and information needs by providing informative, non-clinical information to patients and families, orienting patients/visitors to the hospital, unit, patient room services.

· In collaboration with the licensed nurse, reinforces the patient education provided by the nurse and provides patients/families with basic patient care instruction, such as instruction for using the call light and safety requirements.

6. COMPETENCY

· Complies with established practice standards, policies, procedures, protocols, guidelines, and regulations

· Maintains and demonstrates compliance with hospital and nursing competencies and care standards. Maintains competence in relation to a variety of technical skills that may include but are not limited to performing EKGs and blood glucose monitoring, applying simple dressings and Heartsaver CPR and AED skills.

7. SKILLS/COMPETENCIES REQUIRED: Must be realistic, objective, measurable and related to principal duties described above.

Part Time Job Summary: The Registered Nurse (Internal Traveler) assesses, plans, implements and evaluates nursing care, utilizing the nursing process in accordance with hospital standards and unit policies/procedures for the purpose of monitoring and evaluating patient functional status. Minimum Education: Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom. Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered Nurse License. BLS and ACLS preferred. Work Experience: 1 year related experience in selected specialty. Knowledge, Skills and Abilities: Working knowledge of disease state management processes, bio- psychosocial model, and clinical pathways. Effective interpersonal, written and oral communication skills Ability to organize and prioritize work in an effective and efficient manner. Ability to be detail oriented as required in the examination of clinical and numerical data. Essential Functions and Responsibilities: Utilizes the nursing process in caring for assigned patients. Completes assessment and individualizes plan of care within established unit/nursing guidelines. Carries out nursing care with consideration for cultural and psychosocial factors and ages of individual patients. Organizes, prioritizes, and performs nursing skills appropriate for the patient population. Anticipates patient/unit needs. Demonstrates sound knowledge base: Bio-psychosocial. Delegates and communicates effectively and appropriately. Demonstrates professional judgment in response to changing patient/unit situations. Documents steps of the nursing process/care on the appropriate record. Records data concisely, consistently, accurately, and legibly. Utilizes correct medical terminology abbreviations. Addresses problems and progress toward outcomes according to unit/nursing policy. Facilitates patient teaching discharge. Makes timely, appropriate referrals. Utilizes resources in planning and presenting patient education, considering patient age and cultural/psychosocial factors. Collaborates with other disciplines as appropriate. Supports unit/hospital goals through PI and educational activities. Attends mandatory in-services and meets unit education standards. Accurately completes variance reports. Decision Making: Independent judgment in planning the sequence of operations and making minor decisions in a complex technical or professional field.
