Position Summary:
As a member of the skilled nursing team, the Licensed Practical Nurse, under the direction and supervision of the Registered Nurse, is responsible for giving resident care using the nursing process and ensuring residents receive care outlined in the nursing care plan. Services to be provided for residents over 16 years of age regardless of age, race, religion, and cultural background. Majority of services provided for the elderly.
Education, License & Cert:
State of Pennsylvania LPN License.
Experience:
Preferable but not required. Maintains CPR certification. Ability to read, write, and understand written and oral communication. Follows universal precautions and utilizes proper body mechanics. Keeps up to date in the field of nursing, especially geriatrics. Act 34 clearance (criminal background check).
Essential Functions:
1. Compliance with rules and regulations of all federal, state, and local governments is a requirement of employment.
2. Meets with the Director of Nursing to plan unit work routines, implementation of policies/procedures and to discuss unit needs/problem solving.
3. Assists in orienting new personnel.
4. Keeps RN/supervisor informed of activities on unit.
5. Supervises nursing assistants.
6. Makes rounds of residents, observes and evaluates resident’s needs, condition, and documents care given.
7. Provides direct resident care.
8. Assists with resident and family teaching.
9. Provides input into resident care plans.
10. Maintains residents’ rights as established.
11. Assumes other responsibilities as assigned, reasonably related.
12. Keeps informed of fire, safety, and disaster programs.
13. Attends designated in-service programs.
Other Duties:
1. Able to interact and direct staff in a positive and professional manner.
2. Attends staff meetings.
3. Participates in Performance Improvement activities.
4. Participates in team conferences.
5. Other duties as assigned.
