This position is full time, day hours, working three 12-hour shifts, 7am-7pm and occasional weekends. This is a hybrid position working at Carilion Roanoke Memorial Hospital (CRMH) ED, with 1 day remote.

The Clinical Coordinator uses advanced nursing processes in decision-making, patient assessment, data analysis, development and implementation of methods and systems to improve the overall quality and efficiency of patient care for the designated department and population served. Provides direct and indirect patient care as needed; oversees the delivery of patient care; and provides day to day management of programs to coordinate patient care as designated by the department. Coordinates the delivery of safe, high quality patient care; identifies areas for study; and participates in and/or supports research. Works in compliance with all State, Federal, and other regulatory requirements. Serves as a resource and mentor to staff and team members.

  • Organizes, formulates, and coordinates a multi-disciplinary approach to the patient population for achievement of optimum outcomes.
  • Assesses, implements, and coordinates patient care plans  including, but not limited to: assess referrals to ensure appropriateness of   admission/appointment; criteria is met; patient discharge ongoing care with documentation of patient education and instructions; screening of referrals to ensure appropriate patient placement in programs; ensuring patients and/or family members understand the plan of care;  preparing the patient and family for ongoing post discharge care; and responding to patients’ needs and concerns.
  • Problem solves to facilitate effective outcomes and resolves complaints, issues, or concerns.
  • Plans, implements, and coordinates strategies through evidence-based practices to support quality patient care; and improve outcomes (e.g., length of stay, turn-around time, resource consumption).
  • Develops, collects, analyzes, and evaluates data as needed to identify trends. Presents findings and makes recommendations for performance and quality improvement, JCAHO/regulatory agency requirements, and tracking patient outcomes aimed at accomplishing department and organization goals.
  • Serves as advocate for patient, protector of patient’s rights, and provider of psychological/social/cultural support.
  • Integrates patient’s individual religious/ spiritual, ethnic, and cultural beliefs into the plan of care.
  • Develops, implements, coordinates, and provides continuing education for the department, staff, patients, and families, as well as protocols to support standardization of care.
  • Assesses and provides education. Education is specific to the needs of the patient and well documented.
  • Education is incorporated into the plan of care and includes physicians, staff, and population specific representatives about patient care options.
  • Serves as a liaison between department staff, patients and families, physicians, external vendors/partners to ensure a collaborative approach to care for patients and families.
  • Interacts with staff, patients, physicians, families, visitors, and public, representing Carilion Clinc during all interactions.
  • Age specific admission and ongoing assessments are completed, documented, and used to develop a plan of care.
  • Coordinates discharge needs of the patient to social work to prepare for discharge.
  • Recommend different general statement like: “
  • Coordinates discharge / ongoing care needs with ancillary services as appropriate.”
  • Communicates overall plan of care to direct care nursing staff, as well as changes in plan of care during the course of treatment.  
  • Acts as a resource for peers, others in the organization, families, and community members.
  • Organizes educational presentations, exhibits, and community events for professionals and the community; and represents Carilion Clinic at conferences and professional meetings.
  • Promotes programs and arranges exhibits at various professional presentations, health and career fairs and community events in collaboration with the management team and Strategic Development.
  • Performs office functions as defined by the department including but not limited to scheduling appointments; answering calls; collecting, maintaining, and analyzing data; identifies trends and develops documents, spreadsheets, and other tools as applicable to the department population.
  • Supports the orientation of new employees; assesses and implements unit educational needs and performs other duties as designated by the department.
  • Participates in department and system performance improvement initiatives.
  • Uses analytical thinking to evaluate the effect of care coordination and interventions on quality outcomes.
  • Performs concurrent medical record reviews in assigned area. Collects and analyzes relevant patient data in accordance with accepted Disease Guidelines. Runs outlier reports for ongoing chronic care patients. Analyzes and evaluates the effect of care management on quality outcomes and fiscal parameters.
  • Documents utilization review in accordance with departmental guidelines. (Database/Disease registry management).
  • Actively participates in quality improvement projects.

Facilitates daily coordination of patient care. Interacts with staff, patients, families, visitors, members of the health care team and external partners  to develop, coordinate, and manage the plan of patient care. Demonstrate  knowledge of the principles of growth and development and possess the ability to collect and assess data of patient populations. Must work autonomously  and in a team environment. Utilize delegation and critical thinking skills to provide quality service through a team oriented,  collaborative approach to patient care. Demonstrate  the role of patient advocate, protector of patient’s rights, provider of psychological/social/cultural support, and  tolerate stressful situations. Maintain current knowledge and compliance with accrediting agencies (Joint Commission) within the health care environment, and  state and federal regulations. Performs work in a cost-effective manner keeping cognizant of patient/family needs and the needs of the organization.

Performs in accordance with departmental policies and procedures. May be exposed to infectious diseases, biohazards, and hazardous materials, including regulated medical waste and combative patients. May have 24x7 responsibility. Regular travel for role specific education and coordination of services may be required.

What We Require:

Education: Graduate of an accredited School of Nursing required. Bachelor’s degree Preferred.

Experience: Three years of RN experience required. At least 2 years of experience in relevant specialty or department, as defined by the department, desired. (ex: hospital setting, vascular, rehab, long term care, home health, cardiology, ambulatory, surgery)

Licensure, certification, and/or registration: Current VA RN license required.

Must obtain & maintain appropriate certifications as defined by the department: ACLS, Conscious Sedation, CCRN, ONC, Pain Management and other modality specific certifications (ex. cardiology, surgery, orthopedics, vascular) preferred.

Valid Driver's license required for employees working in Home Health or departments where the work requires travel.

Life Support: AHA BLS-HCP required within 1 year of hire.

Other Minimum Qualifications:  Effective interpersonal, communication, and team-working skills required. Strong critical thinking, conflict resolution, organization and effective judgement abilities required. Must successfully complete orientation and competency validation requirements. Must be able to meet essential job functions. Designated areas may require Knowledge of Medicare/Medicaid and other third-party payor regulations.
