Responsibilities

  • Provide direct nursing care to residents according to individualized care plans
  • Administer medications and treatments as prescribed
  • Monitor resident conditions and report changes promptly
  • Complete accurate documentation in compliance with NYS regulations
  • Collaborate with RNs, CNAs, and the interdisciplinary care team
  • Maintain resident safety, dignity, and comfort

Requirements

  • Valid New York State LPN license
  • Strong communication and organizational skills
  • Ability to work effectively as part of a healthcare team
  • Compassionate and reliable work ethic
  • Nursing home or long-term care experience preferred (not required)
